- HMO plans generally ask you to use in-network providers and pick a primary care doctor, with referrals for specialists.
- HMO-POS (Point of Service) plans work like an HMO but allow some out-of-network care, usually at a higher cost.
- PPO plans let you see out-of-network providers, usually at a higher share of cost, and typically do not require referrals.
- D-SNP (Dual Eligible Special Needs Plan) is for people who have both Medicare and Medicaid.
