The widest reach belongs to Aetna Medicare Longevity (PPO I-SNP), listed in 62 counties — an Institutional SNP, so eligibility is limited. Aetna Medicare Eagle Giveback (PPO) appears in 53 counties. Two chronic-condition plans — Aetna Medicare Chronic Care Total (HMO C-SNP) and Aetna Medicare Full Dual (HMO D-SNP) — are each listed in 45 counties, though both are SNPs that only accept members who meet their eligibility rules.
Among the general-enrollment HMO and PPO options, Aetna Medicare Signature (HMO), Aetna Medicare Signature (HMO) (new for 2027) and Aetna Medicare Elite (PPO) (new for 2027) reach the most counties. What is new or not returning
Year over year, Aetna is adding 10 new plans in New York for 2027 and discontinuing 10 plans that will not return. New-for-2027 entries include additional versions of Aetna Medicare Elite (HMO), Aetna Medicare Elite (PPO), Aetna Medicare Signature (HMO), and both Aetna Medicare Chronic Care (HMO C-SNP) and Aetna Medicare Chronic Care Total (HMO C-SNP).
Two plans are being renamed rather than ended: one Aetna Medicare Enhanced (PPO) was previously called Aetna Medicare Premier (PPO), and the plan now called Aetna Medicare FIDE (HMO D-SNP) was previously Aetna Medicare FIDE Care (HMO D-SNP). A rename can still come with other changes, so the Annual Notice of Change (ANOC) your plan mails each fall is the document to read closely.
If your current Aetna plan is not on the 2027 list, your coverage under that specific plan ends December 31, 2026, and you will need to pick a new plan during the Annual Enrollment Period if you want Medicare Advantage coverage to continue without a gap.
Checking whether a plan is offered in your county
A plan listed statewide may not be offered in your county. Before you count on any Aetna plan, confirm three things:

Use the plan's online provider directory, or call Aetna member services using the number on the plan's page, to verify that your providers are in network for 2027. 3. Prescriptions. Two plans with similar names — for example, several versions of "Aetna Medicare Signature (HMO)" or "Aetna Medicare Elite (PPO)" — can have different county footprints, networks and drug lists. Match the plan ID shown on medicare.gov, not just the name.
What to do before December 7
The Annual Enrollment Period runs October 15 – December 7, 2026. A choice made during that window takes effect January 1, 2027. A practical checklist:
- Read your current plan's Annual Notice of Change, which arrives by late September 2026.
- Make a list of your doctors, preferred hospitals and current prescriptions (with dosages).
- Compare plans side by side at medicare.gov using your ZIP code.
- If you qualify, consider whether a D-SNP, C-SNP or I-SNP fits your situation — these have specific eligibility rules.
- Confirm your enrollment in writing or by confirmation number.
If December 7 passes and you want to switch Medicare Advantage plans, the Medicare Advantage Open Enrollment Period (January 1 – March 31, 2027) allows one change for people already in a Medicare Advantage plan.
