What Devoted Health offers statewide in 2027
Across North Carolina, Devoted Health's 37 plans break down into 28 HMOs and 9 PPOs. Every one of the 37 plans includes Part D prescription drug coverage, so members would not need a stand-alone drug plan alongside them.
Thirteen of the 37 are Special Needs Plans (SNPs), which are limited to people who meet specific eligibility rules. Devoted Health is offering 3 Dual Eligible Special Needs Plans (D-SNPs) for people with both Medicare and Medicaid, and 10 Chronic Condition Special Needs Plans (C-SNPs) for people living with certain qualifying conditions. There are no Institutional SNPs (I-SNPs) in Devoted Health's North Carolina lineup for 2027.
Devoted Health is one of 16 carriers participating in the Medicare Advantage market in North Carolina for 2027. The 15 other carriers together with Devoted Health account for the state's 179 total Medicare Advantage plans.
Plans with the widest county reach
The table on this page lists all 37 Devoted Health plans and the number of counties where each is offered for 2027. A few reach across much of the state:
- DEVOTED DUAL 009 NC (HMO D-SNP), DEVOTED DUAL FULL 013 NC (HMO D-SNP), and DEVOTED DUAL PLUS 006 NC (HMO D-SNP) are each offered in 52 counties.
- DEVOTED C-SNP PLUS 015 NC (HMO C-SNP) is also offered in 52 counties.
- Among the PPOs, DEVOTED CHOICE MA ONLY 007 NC is offered in 49 counties.
Other plans have narrower footprints. Several of the HMO C-SNPs, HMO giveback variants, and Core HMO plans appear in just a handful of counties. "Widest reach" is only a measurement of county count — it is not a quality judgment, a ranking, or a recommendation. Whether any plan is a fit depends on where you live, who your doctors are, and which prescriptions you take.
What's new, what's changing, and what's not returning
Of Devoted Health's 37 plans for 2027, 17 are new for next year. These include several Core HMO plans (such as DEVOTED CORE 021, 023, 029, and 037 NC), several Giveback HMO plans (such as DEVOTED GIVEBACK 022, 024, and 030 NC and DEVOTED GIVEBACK EXTRAS 038, 039, 041, 042, and 044 NC), and new HMO C-SNP options under the Giveback Extras label (DEVOTED C-SNP GIVEBACK EXTRAS 031, 032, 033, 034, and 045 NC).
Four C-SNPs carried over from 2026 have been renamed from "Premium" to "Enhanced": DEVOTED C-SNP ENHANCED 014, 016, 017, and 018 NC. These are the same plan contracts under new names, per the CMS files.
No Devoted Health plans in North Carolina are leaving the market for 2027. That is useful context for current members, since there is no plan termination to react to, but it does not mean plan details stay the same — those can change from year to year.
Checking whether a plan is offered in your county — and covers your doctors and drugs
Statewide counts answer one question only: how many plans exist on paper. They do not tell you what is sold on your street. To check your own county:
1. Go to medicare.gov and open the Medicare Plan Finder. 2. Enter your ZIP code and, if you want, your prescriptions and preferred pharmacy. 3. Filter to Medicare Advantage plans and, if you want, to Devoted Health. 4. Open each plan you're considering and look at the Summary of Benefits, the provider directory, and the formulary (drug list).
For doctor and hospital questions, call the plan directly or check its online directory. For prescriptions, compare each drug on the plan's formulary — tiers and coverage rules can differ between plans with similar names. The Evidence of Coverage, available from the plan, is the authoritative document for how a plan works.
What to do before December 7
The Annual Enrollment Period runs October 15 through December 7, 2026. Any Medicare Advantage choice you make during that window takes effect January 1, 2027.
A few steps that tend to help:
- Make a short list of your doctors, hospitals, and prescriptions before you compare plans.
- If you're already in a Medicare Advantage or Part D plan, read your Annual Notice of Change, which arrives in the fall and spells out what's changing for 2027.
- If you may qualify for a D-SNP or a C-SNP, confirm your eligibility before enrolling, since those plans require specific qualifications.
- Write down any confirmation numbers from enrollments or disenrollments.
People who qualify for a Special Enrollment Period — for example, after a move or a change in Medicaid status — may have additional windows outside of October 15 through December 7, 2026.
