CINCINNATI — As the Annual Enrollment Period for 2027 coverage opens October 15, 2026, Medicare Advantage shoppers in Hamilton County will see a reshuffled menu: 30 plans that were sold here in 2026 will not return next year, 16 new plans are coming in, and 61 plans continue. That is the picture emerging from CMS's 2027 Medicare Advantage Landscape data for the county that includes Cincinnati.

The headline change is concentrated in dual-eligible and other Special Needs Plans, where several big carriers are retiring plan designs and launching replacements. If you are already enrolled in a plan that is leaving, you have options — and a special window to use them.

What is changing in Cincinnati's Medicare Advantage lineup for 2027?

Cincinnati is in Hamilton County, where CMS's 2027 Landscape file shows 30 Medicare Advantage plans leaving after 2026 and 16 new plans arriving January 1, 2027. Another 5 plans are keeping the same contract but changing names, and 21 continuing plans have at least one change on premium, deductible, type or name — details that will be in each plan's Annual Notice of Change (ANOC) mailed to current members.

The table on this page lists every plan offered in the county for 2027, grouped by carrier.

Which Medicare Advantage plans are not returning to Hamilton County in 2027?

Thirty plans sold in Hamilton County in 2026 are not being offered for 2027. By carrier, they are:

If a plan name here looks familiar because a similar one is launching for 2027, that is not a coincidence — several carriers are retiring and relaunching dual-eligible plans under new contracts.

Which Medicare Advantage plans are new for 2027 in Hamilton County?

Sixteen Medicare Advantage plans are new to Hamilton County for 2027. By carrier:

A handful of 2026 plans are also being renamed rather than replaced, including Wellcare Buckeye MyCare Ohio Dual Align Unity (HMO D-SNP), DEVOTED C-SNP ENHANCED 018 OH (HMO C-SNP), Anthem I MyCare Ohio Full Dual Support (HMO D-SNP), Humana Full Access (PPO), and MedMutual Advantage Classic (HMO-POS). Members of a renamed plan generally stay enrolled and will see the new name on their 2027 materials.

Who sells Medicare Advantage in Cincinnati, and what plan types are available?

Hamilton County residents can choose among 14 carriers for 2027-year reference figures from the current CMS Landscape file. The current-year totals show the county with 96 Medicare Advantage plans — 40 HMOs, 18 HMO-POS, 37 PPOs and 1 PFFS — of which 34 are Special Needs Plans and 86 include Part D drug coverage.

Carriers serving the county include Aetna (CVS Health), Blue Cross Blue Shield (HCSC), CareSource MyCare Ohio, Centene (WellCare), Communicare Advantage, Devoted Health, Elevance Health (Anthem), Humana, Medical Mutual of Ohio, MediGold, Molina Healthcare of Ohio, Perennial Advantage, UnitedHealthcare, and Valor Health Plan. The table below lists every plan offered for 2027 under each carrier.

What happens if my Medicare Advantage plan is leaving Cincinnati?

If your plan is on the leaving-after-2026 list, your coverage ends December 31, 2026, and you qualify for a Special Enrollment Period that lets you choose a different Medicare Advantage plan, join a stand-alone Part D plan, or return to Original Medicare. Your plan is required to send you a non-renewal notice this fall explaining the end date and your options.

You do not have to wait for that notice to act. During the Annual Enrollment Period (October 15 – December 7, 2026), you can simply pick a new plan the ordinary way, and the new coverage starts January 1, 2027. If you miss the AEP deadline, the non-renewal Special Enrollment Period gives you additional time into early 2027 to make a selection and keep continuous coverage.

If you do nothing and your current plan is continuing, it will roll over automatically for 2027 with the changes described in its Annual Notice of Change — worth a careful read.

What else is changing across Medicare for 2027?

Two Part D figures are already confirmed by CMS for 2027. The annual out-of-pocket cap on covered Part D drugs rises to $2,400 (from $2,100 in 2026), and the maximum Part D deductible a plan can charge rises to $700 (from $615 in 2026). The Part D national base beneficiary premium for 2027 is $41.33 — that is a formula input CMS uses, not the premium any particular plan charges.

The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not yet been announced. CMS typically releases those figures later in the fall, usually in November. The Medicare Prescription Payment Plan — the option to spread drug costs across monthly payments — continues to be available for 2027.

Good to know

Frequently asked questions

When do changes I make this fall take effect?
Any Medicare Advantage or Part D change you make during the Annual Enrollment Period, which runs October 15 through December 7, 2026, takes effect January 1, 2027.
How will I know if my plan is leaving for 2027?
Your plan is required to mail a non-renewal notice this fall if it will not be offered in 2027. If your plan is continuing but changing, you will instead receive an Annual Notice of Change (ANOC) describing what is different for 2027.
Do I have to do anything if I like my current plan?
No. If your plan is being offered again in 2027, it renews automatically with the changes outlined in its ANOC. Reviewing that document each year is a good habit, especially since premiums, deductibles, drug lists and provider networks can change.
Where can I compare the specific plans available in Cincinnati?
Use the Medicare Plan Finder at medicare.gov to compare plans by ZIP code, or call 1-800-MEDICARE (TTY 1-877-486-2048), available 24 hours a day, seven days a week, for personalized help. For official plan details, costs and provider networks, check each plan's own documents, use the Medicare Plan Finder at medicare.gov, or call 1-800-MEDICARE.
