What "carrier by carrier" tells you
Medicare Advantage plans are sold by private insurance companies that contract with Medicare. In Lucas County, 14 carriers are offering plans for 2027, with 19 new plans on the roster, 31 not returning from 2026 and 4 renamed. MediGold and Paramount Elite Medicare Plans are not offering plans in the county next year. Carriers continuing to sell plans include Humana, UnitedHealthcare, Devoted Health, Aetna (CVS Health), Elevance Health (Anthem), CareSource MyCare Ohio, Communicare Advantage, Molina Healthcare of Ohio, Valor Health Plan, Medical Mutual of Ohio, Perennial Advantage, SummaCare, Centene (WellCare) and Zing Health.

Plan types available in Toledo
Every Medicare Advantage plan on the Lucas County list falls into one of three network types:
- HMO (Health Maintenance Organization): 37 plans. HMOs generally ask you to use in-network providers and pick a primary care doctor.
- HMO-POS (Point of Service): 26 plans. HMO-POS plans work like an HMO but allow some out-of-network care under specific rules.
- PPO (Preferred Provider Organization): 36 plans. PPOs typically let you see out-of-network providers, usually at a higher cost share.
Some plans also carry a suffix like C-SNP, D-SNP or I-SNP. Drug coverage and the 2027 Part D rules
All 83 Medicare Advantage plans available in Lucas County for 2027 include prescription drug coverage — often called MA-PD plans. Dates to keep on your calendar
- Oct. 15 – Dec. 7, 2026: Medicare Annual Enrollment Period. You can join, switch or drop a Medicare Advantage or Part D plan for 2027.
- Jan. 1 – March 31, 2027: Medicare Advantage Open Enrollment. If you are already in a Medicare Advantage plan on Jan. 1, you get one chance to switch to another MA plan or return to Original Medicare (and add a stand-alone Part D plan).
- Special Enrollment Periods may apply if you move, lose other coverage or qualify for certain programs like Extra Help or Medicaid.
