How the plan types differ
Medicare Advantage plans in Pittsburgh come in four shapes for 2027.
HMO (40 plans). An HMO uses a defined network of doctors and hospitals. You usually pick a primary care doctor, and you generally need a referral to see a specialist. Out-of-network care is typically not covered except in emergencies.
HMO-POS (19 plans). A "point-of-service" HMO works like an HMO but adds a limited ability to go out of network for certain services, usually at higher cost sharing.

If you travel often or split time between states, that HMO-vs-PPO choice tends to matter more than the carrier's name. If you rely on a specific health system, it is worth checking which networks include your doctors before comparing anything else.
Special Needs Plans: who they are for
Of the 96 plans in the county, 33 are Special Needs Plans (SNPs). These are Medicare Advantage plans limited by law to specific groups:
- D-SNP (Dual-Eligible): for people who have both Medicare and Medicaid.
- C-SNP (Chronic Condition): for people with certain qualifying chronic conditions.
- I-SNP (Institutional): for people who live in a nursing facility or need that level of care at home.
You cannot simply pick an SNP because it looks appealing — you have to qualify. If you do qualify, an SNP is often designed around the care you already need.
