Three things to know when you compare your 2026 plan to the 2027 lineup:
- One Humana plan is not returning in Travis County for 2027. If your plan is the one being discontinued, Humana was required to send you a non-renewal notice, and you have a Special Enrollment Period to pick a new plan.
- One Humana plan is new for 2027 — an additional Humana Together in Health (PPO I-SNP) offering, which is a type of plan designed for people who need an institutional level of care.
- Four plans have new names. Humana Essentials Plus Giveback (PPO), HumanaChoice Giveback H5216-358 (PPO), HumanaChoice H5216-432 (PPO), and HumanaChoice H7617-029 (PPO) all carry the same contract numbers as 2026 plans with different names. A rename does not automatically mean the benefits are identical — read the ANOC.
Who the Special Needs Plans are for
Four of Humana's 17 plans are SNPs, and these are not for everyone. They have eligibility rules:
- D-SNP (Dual Eligible): Humana Gold Plus SNP-DE H0028-032 and Humana Gold Plus SNP-DE H4461-071 — for people who have both Medicare and Texas Medicaid.
- C-SNP (Chronic Condition): Humana Gold Plus - Diabetes and Heart (HMO C-SNP) — for people with qualifying diabetes and/or cardiovascular conditions.
- I-SNP (Institutional): Humana Together in Health (PPO I-SNP) — for people who live in or need a nursing-home level of care.
If you don't meet the eligibility rules, you cannot enroll in these plans, even if you like what you see.
How to check a plan against your doctors and drugs before December 7
This is the step most people skip, and it's the one worth doing before you lock in a choice for 2027:
1. Read your Annual Notice of Change. Humana mails it each September. It shows what's changing in your current plan for 2027 — premium, deductible, drug tiers, copays, extras. 2. Use the Medicare Plan Finder at medicare.gov. Enter your ZIP code (Austin ZIPs are in Travis County), add your prescriptions by name and dose, and add your preferred pharmacies. The tool shows total estimated yearly cost for each plan, including drugs. 3. Check your doctors. Plan Finder links to each plan's provider directory. For HMO plans, in-network care generally matters more than for PPO plans — confirm your primary care doctor and any specialists are in-network for 2027, not just 2026. 4. Confirm the pharmacy you use. Preferred pharmacies usually cost less than standard in-network ones, and that can change year to year. 5. A few program-level numbers to anchor your planning:
- Part D out-of-pocket cap: $2,400 in 2027 (up from $2,100 in 2026). Once your out-of-pocket spending on covered drugs hits that cap, you pay $0 for covered drugs for the rest of the year.
- Part D maximum deductible: $700 in 2027 (up from $615 in 2026). Plans can set a lower deductible, but not higher.
- The 2027 Part B standard premium, Part B deductible and IRMAA income brackets have not been announced yet. CMS usually publishes them in November.
If you do nothing
Your current plan renews on January 1, 2027, with the changes in its ANOC — unless it's the one Humana plan not returning in Travis County, in which case Humana will have notified you and given you a Special Enrollment Period. After December 7, Medicare Advantage members get a second window — the Medicare Advantage Open Enrollment Period, January 1 to March 31, 2027 — to switch to another Advantage plan or return to Original Medicare.
