What's new, what's not returning, what's renamed
CMS's year-over-year comparison for Dallas County shows a modest reshuffle inside Humana's book of business:
- 5 new plans for 2027. Two of them are branded Humana DaVita Kidney Care (PPO C-SNP) — Chronic Condition SNPs that serve people with kidney disease. The others include a new Humana Gold Plus – Diabetes and Heart (HMO C-SNP), a new Humana Together in Health (PPO I-SNP), and a new Humana Value Choice (PPO).
- 4 plans not returning in 2027. If you're in one of those, Humana was required to send you a non-renewal notice. You have a Special Enrollment Period to pick a different plan.
- 2 renamed plans. Humana Essentials Plus Giveback (PPO) was previously Humana Essentials Plus Giveback H7617-035 (PPO). Humana Total Complete (HMO) was previously Humana Total Complete H4461-051 (HMO). Same contract, new display name.

About Special Needs Plans (SNPs)
Humana's 2027 Dallas County roster includes three kinds of SNPs. These are Medicare Advantage plans built around a specific group of people:
- D-SNPs are for people who have both Medicare and Medicaid.
- C-SNPs are for people with specific chronic conditions, such as diabetes, heart disease or chronic kidney disease.
- I-SNPs are for people living in a nursing facility or who need that level of care at home.
