Reading the plan types
Every 2027 Medicare Advantage plan in Collin County falls into one of four types, and the type tells you a lot about how you'll use the plan day to day.
HMO (Health Maintenance Organization). You pick a primary care doctor and stay in-network except in emergencies. Referrals are usually needed for specialists. Thirty-seven of the county's plans are HMOs.
HMO-POS (HMO with a Point-of-Service option). Like an HMO, but with limited coverage for some out-of-network services. Twenty-six plans are HMO-POS.
PPO (Preferred Provider Organization). More flexibility to see out-of-network providers, usually at a higher cost share. Thirty-two plans are PPOs, including some "Regional PPO" filings.
PFFS (Private Fee-for-Service). You can see any Medicare-approved provider that agrees to the plan's terms. There are 2 PFFS plans in the county — both from Humana.
What "SNP" means on the list
Thirty-eight of the county's 97 plans are Special Needs Plans, or SNPs. They're regular Medicare Advantage plans, but only for people who meet the specific eligibility rules:
- D-SNP — for people who have both Medicare and Medicaid ("dual eligibles"). Examples in Collin County include Aetna Medicare Dual Care, Blue Cross Medicare Advantage Dual Care Plus, HealthSpring TotalCare, Wellpoint Dual Advantage, Molina Medicare Complete Care and several UHC Dual Complete plans.
- C-SNP — for people with certain chronic conditions such as diabetes, heart disease or kidney disease. Examples include Aetna Medicare Prime Chronic Total, Humana Gold Plus – Diabetes and Heart, Wellpoint Kidney Care, ProCare Advantage – Kidney Care and Verda Noble Chronic Care.

Drug coverage is built in — but the Part D rules still apply
