Garland, Texas — Fall 2026. As Medicare's Annual Enrollment for 2027 approaches, people in Garland living with a long-term health condition have a specific type of Medicare Advantage plan to consider: a Chronic Condition Special Needs Plan, or C-SNP. These plans are built around a qualifying diagnosis, and enrollment comes with its own rules. Here is how they work in Dallas County, the county Garland is in, for the coverage year starting January 1, 2027.
What is a C-SNP and how does it work in Garland for 2027?
A C-SNP is a type of Medicare Advantage plan designed for people with a specific severe or disabling chronic condition — for example, diabetes, chronic heart failure, or end-stage renal disease. In Dallas County for 2027, 15 of the 84 Medicare Advantage plans are C-SNPs. The other Special Needs Plans in the county are 20 D-SNPs (for people with both Medicare and Medicaid) and 7 I-SNPs (for people in an institutional setting like a nursing home).
Because a C-SNP is a Medicare Advantage plan, it replaces Original Medicare for your Part A and Part B services, and most C-SNPs include Part D prescription drug coverage. What makes a C-SNP different is that its provider network, drug list, and care coordination are shaped around the condition it serves. CMS requires every C-SNP to have a model of care tailored to its members.
Which carriers offer C-SNPs in Dallas County for 2027?
Six carriers offer C-SNPs in Dallas County for 2027. In alphabetical order, they are Aetna (CVS Health) with 1 plan, Elevance Health (Anthem) with 4, Humana with 5, ProCare Advantage with 2, UnitedHealthcare with 2, and Verda Health Plan of Texas with 1.
The full list of C-SNPs available in the county for 2027, in alphabetical order by carrier, is:
- Aetna Medicare Chronic Care Total (HMO C-SNP) — Aetna (CVS Health); renamed from Aetna Medicare Prime Chronic Total (HMO C-SNP)
- Wellpoint Chronic Care (HMO-POS C-SNP) — Elevance Health (Anthem)
- Wellpoint Chronic Care Advantage Select (HMO C-SNP) — Elevance Health (Anthem); new for 2027
- Wellpoint Chronic Care Select (HMO-POS C-SNP) — Elevance Health (Anthem); new for 2027
- Wellpoint Kidney Care (HMO-POS C-SNP) — Elevance Health (Anthem)
- Humana DaVita Kidney Care (PPO C-SNP) — Humana; new for 2027
- Humana Gold Plus - Diabetes and Heart (HMO C-SNP) — Humana (one of these is new for 2027)
- ProCare Advantage - Diabetes Care Management (HMO-POS C-SNP) — ProCare Advantage
- ProCare Advantage - Kidney Care (HMO-POS C-SNP) — ProCare Advantage
- UHC Complete Care TX-29 (Regional PPO C-SNP) — UnitedHealthcare
- UHC Complete Care TX-3P (HMO-POS C-SNP) — UnitedHealthcare
- Verda Noble Chronic Care (HMO C-SNP) — Verda Health Plan of Texas
Only plans whose names state the condition they serve (such as "Kidney Care" or "Diabetes and Heart") can be assumed to serve that condition; for the others, the plan documents and the Medicare Plan Finder will tell you which diagnoses qualify.
How do you qualify to join a C-SNP in Garland?
You qualify for a C-SNP only if you have the specific chronic condition the plan is built around, and your diagnosis must be confirmed by a health care provider. When you apply, the plan will reach out to your doctor to verify the condition. If the diagnosis cannot be confirmed within a set period, the plan has to disenroll you.
That means two things for Garland residents. First, before you apply, make sure your condition is clearly documented in your medical record and that your provider is willing to confirm it to the plan. Second, pick a C-SNP that actually serves your condition — a diabetes-and-heart plan will not accept someone whose only qualifying condition is end-stage renal disease.
When can you enroll — and what about the Special Enrollment Period?
Most people enroll during the Annual Enrollment Period, which runs October 15 through December 7, 2026, for coverage that starts January 1, 2027. But people with a qualifying chronic condition also get a Special Enrollment Period (SEP) to join a C-SNP. You can use the SEP to enroll in a C-SNP once outside the Annual Enrollment window after you are first diagnosed; the SEP ends when you enroll or when a set period passes.
If you do nothing during Annual Enrollment, your current plan will renew for 2027 if it is still offered, with any changes spelled out in the Annual Notice of Change (ANOC) your plan mailed in September. In Dallas County, 29 plans are not returning for 2027, 20 are new, and 9 have been renamed — so it is worth opening the ANOC even if you are happy with your current coverage.
What else should Garland residents know for 2027?
A few program-level facts apply to any Medicare Advantage plan with drug coverage, including C-SNPs. For 2027, the Part D out-of-pocket cap rises to $2,400 (up from $2,100 in 2026), and the Part D maximum deductible is $700 (up from $615 in 2026). The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet; CMS typically publishes them in November.
