What the plan types mean
The letters after each plan name tell you how the plan works day to day.
- HMO (Health Maintenance Organization): You generally pick a primary care doctor and stay in the plan's network, except for emergencies.
- HMO-POS (Point of Service): An HMO with a limited option to see some out-of-network providers, usually at a higher cost.
- PPO (Preferred Provider Organization): More flexibility to see out-of-network doctors, usually with higher cost-sharing when you do.
- PFFS (Private Fee-for-Service): The plan sets the terms of payment; providers decide, visit by visit, whether to accept those terms.
- SNP (Special Needs Plan): Limited to people who fit a category — such as being on both Medicare and Medicaid (D-SNP), living in an institution (I-SNP), or having certain chronic conditions like diabetes or kidney disease (C-SNP).
Of the 97 plans on the Collin County list, 38 are Special Needs Plans. You cannot enroll in an SNP unless you meet its eligibility rules.
Drug coverage and the 2027 Part D rules
Every 2027 Medicare Advantage plan on the McKinney/Collin County list includes Part D prescription drug coverage. A few program-level facts apply no matter which plan you look at:
- The Part D out-of-pocket cap is $2,100 in 2026 and $2,400 in 2027. Once your covered drug costs reach that amount, you pay nothing for covered drugs the rest of the year.

1. Check your doctors and hospitals. Look up each provider you want to keep in a plan's network before anything else. 2. List your prescriptions. If you travel or want more freedom to see specialists without referrals, a PPO or HMO-POS may fit better than a strict HMO. 4. Check whether an SNP applies to you. If you have Medicaid, live in a nursing facility, or have a qualifying chronic condition, a Special Needs Plan may be an option worth exploring. 5. Look at total yearly cost, not just the premium. Deductibles, copays and the drug tier your medications fall into can matter more than the sticker premium.
