SAN ANTONIO — If you have a Medicare Advantage plan in Bexar County, this fall's mail is worth opening carefully. According to CMS's 2026 and 2027 Medicare Advantage Landscape files, 28 plans offered in the county this year will not be back in 2027, 26 new plans are coming in, and 11 are keeping the same underlying plan but changing the name on the card.

If your plan is leaving

Members whose plans are being discontinued should get a non-renewal notice from their insurer. If that is you, two things to know:

1. You can use the regular Annual Enrollment Period (Oct. 15 – Dec. 7) to pick a new Medicare Advantage plan or return to Original Medicare with a stand-alone Part D plan. 2. You also qualify for a Special Enrollment Period that runs through the end of February. That gives you extra runway if you need it.

If your plan is not on that list, it is either continuing, being renamed, or has other changes noted in your ANOC.

New plans coming to the county in 2027

Alignment Health Plan is new to Bexar County next year with a set of HMO-POS plans: Alignment Health BRIDGE 013, Alignment Health Duals+ 007, Alignment Health Heart & Diabetes 002, Alignment Health Heart & Diabetes Plus 004, Alignment Health ONE 011 and Alignment Health Smart 012.

Existing carriers are also adding plans. From Aetna (CVS Health): Aetna Medicare Chronic Care Total (HMO C-SNP) and Aetna Medicare QMB Only Care (HMO D-SNP). From Blue Cross Blue Shield (HCSC): HealthSpring TotalCare Plus (HMO D-SNP). Many of these are Special Needs Plans (SNPs) designed for people with specific chronic conditions, dual Medicare-Medicaid eligibility or nursing home care needs. If you do not qualify for a SNP, those plans are not options for you.
