Three things are worth noting before you start comparing:
- 26 plans are new in Bexar County for 2027.
- 28 plans are not returning. If yours is one of them, your carrier is required to send you a non-renewal notice. You then have a Special Enrollment Period to pick a new plan.
- Alignment Health Plan appears in the county's 2027 lineup; two carriers from 2026 — American Health Advantage of Texas and Wellpoint under Elevance's HMO-POS branding — show different footprints next year. A step-by-step way to compare
Pick two or three plans to compare, not ten. Here is the order that saves time.
1. Read your Annual Notice of Change first. Your current plan mailed this by late September. It shows side-by-side what is changing for 2027 — premium, deductible, drug tiers, networks, extra benefits. If nothing meaningful is changing and you are happy, you may be done. If something matters to you, keep going.
2. Check your doctors and hospitals. Every plan has its own network. Use the plan's own online provider directory or call the plan to confirm each doctor, specialist and hospital you use will be in-network for 2027. HMO and HMO-POS plans generally require you to stay in-network except for emergencies; PPO plans usually let you go out of network at a higher cost; PFFS plans work differently again. Networks can change on January 1, so last year's answer is not this year's answer.
3. Check your prescriptions. Make a list of every drug you take, with the dose and how often. Enter them into the Medicare Plan Finder at medicare.gov/plan-compare. The tool will show whether each drug is on a plan's formulary, what tier it is on, and whether there are restrictions like prior authorization or step therapy.
4. Check your pharmacy. Plans have preferred and standard pharmacies, and your cost at the counter depends on which one you use. If you fill at a specific neighborhood pharmacy or by mail order, confirm it is a preferred pharmacy for the plan you are considering.
5. Add up the whole year, not just the premium. Plan Finder will estimate your total yearly cost based on the drugs and pharmacy you entered. A low premium can come with higher copays, and vice versa.
6. Look at the extras last, not first. Dental, vision, hearing, over-the-counter cards and transportation are real benefits, but they should not drive the decision if the plan does not cover your doctor or your medication.
