SAN ANTONIO — Fall 2026. If you have both Medicare and Texas Medicaid and live in or near San Antonio, your choices for 2027 are coming into focus. In Bexar County, 25 Dual Special Needs Plans (D-SNPs) from 10 carriers will be offered for plan year 2027, according to CMS Medicare Advantage Landscape data. That is a bigger menu than many parts of Texas, and it includes several brand-new plans and a handful of renamed ones.

D-SNPs are a type of Medicare Advantage plan built for people who qualify for both Medicare and Medicaid — often called "dual eligibles." Here is what is on the table for 2027, who can join, and how often you can change your mind.

What a D-SNP is, in plain terms

A Dual Special Needs Plan is a Medicare Advantage plan that only enrolls people who have both Medicare and Medicaid. Every D-SNP in Bexar County includes prescription drug coverage, and plans coordinate with Texas Medicaid so your two sets of benefits work together instead of in parallel. Some D-SNPs are designed for people with full Medicaid; others are built for people whose Medicaid only helps with Medicare costs (for example, the Qualified Medicare Beneficiary, or QMB, group).

If your Medicaid status changes, your eligibility for a particular D-SNP can change too. For 2027, Bexar County will have 25 D-SNPs — part of a broader Special Needs Plan picture that also includes 18 Chronic Condition SNPs (C-SNPs) and 7 Institutional SNPs (I-SNPs). The 10 carriers offering D-SNPs are, in alphabetical order:

Several plans are new for 2027, including Aetna Medicare QMB Only Care, Alignment Health Duals+ 007, Wellcare Superior HealthPlan Dual Liberty, Wellcare Superior HealthPlan Dual Reserve Value, three Wellpoint plans from Elevance Health (Anthem), HealthSpring TotalCare Plus, and four UnitedHealthcare "Dual" plans (TX-V11, TX-Q6, TX-Y1 and the renamed TX-S001). A handful of 2026 plans have been renamed; your Annual Notice of Change (ANOC) will spell out exactly what changed on your plan. The table below lists every D-SNP by carrier.

How often a dual-eligible person can change plans

Everyone with Medicare can make a change during the Annual Enrollment Period, Oct. 15 – Dec. 7, 2026, with coverage starting Jan. 1, 2027. People with Medicare Advantage also have the Jan. 1 – Mar. 31 Medicare Advantage Open Enrollment Period to switch MA plans or return to Original Medicare.

Dual eligibles and people with Extra Help get more flexibility. Under current CMS rules, if you have Medicaid or the low-income subsidy, you have a monthly opportunity to either enroll in an integrated D-SNP or drop Medicare Advantage and return to Original Medicare with a stand-alone Part D plan. In other words, you are not locked in until next October. Rules on these Special Enrollment Periods have been tightening in recent years, so before you make a move, confirm what is currently allowed with 1-800-MEDICARE (TTY 1-877-486-2048), 24 hours a day, 7 days a week.

If you do nothing during the fall window, your current plan simply renews for 2027 — assuming it is being offered next year — with the changes described in your ANOC.

The 2027 Part B standard premium and deductible and the 2027 IRMAA income brackets have not been announced yet; CMS usually publishes them later in the fall. If you are a full dual, Texas Medicaid typically pays your Part B premium, so IRMAA and Part B changes may matter less to your monthly budget — but they still appear on your paperwork.

How to confirm eligibility and compare plans
