For 2027, HealthSpring is adding one plan in Bexar County — HealthSpring TotalCare Plus (HMO D-SNP) — bringing its D-SNP count to two. No HealthSpring plans are leaving Bexar County for 2027.

A few Medicare figures are set by CMS and apply no matter which plan you pick:
- Part D out-of-pocket cap: $2,400 in 2027 (up from $2,100 in 2026).
- Part D maximum deductible: $700 in 2027 (up from $615 in 2026).
- The old coverage gap ("donut hole") has been gone since 2025; Part D now has three phases.
- The Medicare Prescription Payment Plan lets you spread your drug costs across the year in monthly installments if you choose.
The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, had not been announced as of early fall 2026. CMS typically publishes those figures later in the fall.
If you do nothing
Nothing bad happens automatically. If your HealthSpring plan is offered again in 2027 — and all five of its 2026 plans are — you stay enrolled, with the changes described in your ANOC. If you want to switch to another HealthSpring plan, to a different carrier, or back to Original Medicare, you can do that between October 15 and December 7, 2026, and the change starts January 1, 2027. There is also a Medicare Advantage Open Enrollment Period from January 1 through March 31, 2027, for people already in a Medicare Advantage plan who want to make one change.
