How you qualify, and how enrollment works
To enroll in a C-SNP, three things have to line up:
1. You have Medicare Part A and Part B. 2. You live in the plan's service area. 3. You have the chronic condition the plan is built around, and your provider confirms it to the plan.
That last step is specific. The plan will ask your doctor to verify your diagnosis, usually with a short form. If the provider does not confirm the condition within the window CMS allows, the enrollment will not stick.
If you do qualify, you get a Special Enrollment Period tied to the chronic condition. In plain terms: you can enroll in a C-SNP outside the Annual Enrollment Period, once per calendar quarter during the first nine months of the year, and you have a window after your diagnosis is confirmed. You do not have to wait for October 15.
How this fits into the 2027 enrollment season
The Annual Enrollment Period for 2027 coverage runs October 15 – December 7, 2026. A change made in that window takes effect January 1, 2027. If you do nothing, your current plan simply renews for 2027 — assuming it is being offered again — with whatever changes are described in its Annual Notice of Change (ANOC). Read that notice when it arrives.
A few 2027 program-level numbers are already set and worth knowing if you take prescription drugs:
