What IRMAA actually is

IRMAA is not a separate plan or penalty for late enrollment. It's a surcharge the Social Security Administration adds to your Part B and Part D premiums when your income is above a set threshold. Most people pay only the standard premium. A smaller group — generally higher earners — pays that standard amount plus an IRMAA amount tied to their income bracket.

The 2027 income thresholds

For 2026, IRMAA begins once a single filer's MAGI is above $109,000, or above $218,000 for a married couple filing jointly. Income at or below those amounts means you pay only the standard Part B and Part D premiums — no surcharge.

From there, the surcharge steps up through several income tiers. The highest tier applies to single filers with MAGI at or above roughly $500,000 (and married couples at or above roughly $750,000). Each step higher adds a larger dollar amount to both your Part B and your Part D premium. CMS publishes the exact dollar figures each fall at Medicare.gov.

Why the two-year lookback matters

Because 2027 IRMAA is based on 2025 income, a one-time bump — selling a house, cashing out an IRA, taking a large capital gain — can push you into a surcharge bracket even if your current income is much lower.

If a life-changing event has reduced your income since 2024, you can ask Social Security to use more recent figures. Qualifying events include:

The form is SSA-44, "Medicare Income-Related Monthly Adjustment Amount – Life-Changing Event." You file it with Social Security, along with documentation. If approved, they'll recalculate your surcharge.
