What IRMAA actually is

IRMAA is not a separate plan or a penalty for signing up late. It is an extra monthly amount that higher-income Medicare beneficiaries pay for Part B (doctor visits and outpatient care) and Part D (prescription drugs). Everyone on Medicare pays the standard Part B premium. People above the income thresholds pay that standard premium plus an IRMAA add-on.

The Part D piece works a little differently. Your drug plan sets its own premium, and IRMAA is billed separately by Medicare on top of it — usually deducted from your Social Security check or billed directly.

The income thresholds for 2027

For 2026, IRMAA starts once your modified adjusted gross income (MAGI) from your 2024 tax return crosses these lines:

Why the IRS looks back two years

IRMAA is not the only Part D number worth knowing. In 2027, the out-of-pocket cap on covered prescription drugs is $2,400, and the maximum Part D deductible is $700.
