Medicare Part D
2027 IRMAA Thresholds for Medicare Premiums in Fayetteville
FAYETTEVILLE, Ark. — As Medicare's Annual Enrollment Period runs Oct. 15 through Dec. 7, some Northwest Arkansas retirees are learning that what they earned two years ago can quietly push up next year's Medicare premiums. For 2027, higher-income beneficiaries will again pay an Income-Related Monthly Adjustment Amount, or IRMAA, on top of standard Part B and Part D premiums.

- IRMAA surcharges for 2026 kick in when a single filer’s income is above $109,000, or above $218,000 for married couples filing jointly.
- Income above those levels raises Part B and Part D premiums in tiers, with the highest surcharge applying to single filers at or above $500,000 and couples at or above $750,000.
- If a life-changing event like retirement lowered your income, you can ask Social Security to reduce your IRMAA by filing Form SSA-44 with documentation.
- Medicare's Annual Enrollment Period runs through Dec. 7, giving Washington County residents time to compare Part D and Medicare Advantage options while keeping IRMAA in mind.
Fayetteville retirees: your tax return could raise your 2027 Medicare bill
If your income sits near one of the IRMAA thresholds, a small change on your tax return can mean a noticeably bigger monthly bill. Here's how the surcharges work and what to watch for this season.
What IRMAA is — in plain language
Most people pay the standard Part B premium and whatever premium their Part D drug plan charges. But if your income is above a certain level, Social Security adds an extra monthly amount to both. That extra amount is IRMAA.
Two important things to know:
- IRMAA is based on your tax return from two years ago. Your 2027 surcharge is generally based on your 2025 income (specifically, your Modified Adjusted Gross Income, or MAGI).
- It applies to Part B and Part D separately. Even if your Part D plan has a low premium, the IRMAA surcharge is added on top and is usually paid directly to Medicare.
The 2026 income thresholds
For 2026, IRMAA kicks in when a single filer's MAGI is above $109,000, or above $218,000 for a married couple filing jointly. Below those numbers, you pay the standard premiums with no surcharge.
Above the first threshold, surcharges rise in tiers — the higher your income, the higher the extra amount. The top tier applies to single filers with income at or above $500,000 (and married couples at or above $750,000).
You can see the current tier chart on Medicare.gov under "Costs" — it lists the exact dollar amounts for both Part B and Part D at each level.
Why 2027 feels different
A few other 2026 changes make understanding your total drug costs more important than ever:
- Part D has a $2,400 out-of-pocket cap in 2027 on covered prescriptions. Once you hit it, you pay nothing more for the rest of the year on covered drugs.
- In 2027, the Part D deductible can be no higher than $700.
- The old "donut hole" coverage gap is gone. Part D now has three simpler phases: deductible, initial coverage, and catastrophic.
- The Medicare Prescription Payment Plan lets you spread your drug costs into monthly installments instead of paying big amounts at the pharmacy counter.
None of these change IRMAA itself — but they do change the full picture of what Medicare will cost you in 2027.
What Fayetteville residents should do this fall
Fayetteville is in Washington County, where Medicare beneficiaries have a range of stand-alone Part D and Medicare Advantage options available during Annual Enrollment (Oct. 15 – Dec. 7). If you're comparing plans, keep IRMAA in mind:
- Look at your 2024 tax return. If your MAGI was close to $109,000 (single) or $218,000 (joint), you may be near a threshold.
- Remember one-time income events. Selling a home, taking a large IRA withdrawal, or converting to a Roth in 2025 can push you into IRMAA for 2027.
- If your income has since dropped, you may qualify for relief. More on that below.
If a life change lowered your income, you can appeal
Social Security allows you to request a reduction in your IRMAA if you've had a "life-changing event." These include:
- Marriage, divorce, or the death of a spouse
- Work stoppage or reduced hours (retirement is the big one)
- Loss of income-producing property
- Loss of pension income
You file Form SSA-44 with documentation. This is one of the most overlooked steps for newly retired Medicare beneficiaries whose two-year-old tax return doesn't reflect today's reality.

Higher-income surcharges
2026 IRMAA income brackets
If your income is above a threshold, you pay an income-related surcharge (IRMAA) on top of your Part B and Part D premiums. Your 2027 surcharge is based on your 2025 tax return. CMS publishes the 2027 bracket amounts later this fall, usually in November.
| Income — single filer | Income — married, joint | Part B monthly premium | Part D surcharge |
|---|---|---|---|
| $109,000 or less | $218,000 or less | $202.90 | Plan premium only |
| $109,001 – $137,000 | $218,001 – $274,000 | $284.10 | + $14.50 |
| $137,001 – $171,000 | $274,001 – $342,000 | $405.80 | + $37.40 |
| $171,001 – $205,000 | $342,001 – $410,000 | $527.50 | + $60.40 |
| $205,001 – $500,000 | $410,001 – $750,000 | $649.30 | + $83.30 |
| Above $500,000 | Above $750,000 | $689.90 | + $91.00 |
