What IRMAA is — and who it affects

IRMAA stands for Income-Related Monthly Adjustment Amount. It's an extra charge added to the standard Part B premium and, if you have Part D drug coverage, to your Part D premium as well. Most people with Medicare do not pay IRMAA — it applies only to a smaller share of beneficiaries whose income crosses the thresholds set each year.

IRMAA isn't the only thing shifting for 2027.
