Medicare Part D
Meridian, Idaho IRMAA Letters & 2027 Medicare
MERIDIAN, Idaho — If you're on Medicare and your income is above average, a letter from Social Security this fall could catch you off guard. It's called an IRMAA notice — short for Income-Related Monthly Adjustment Amount — and it tells higher-income beneficiaries they'll pay extra for Part B and Part D in 2027. CMS publishes the 2027 thresholds later this fall, so it's worth knowing how the lines are drawn before your first 2027 premium hits.

- IRMAA surcharges kick in for 2026 when a single filer's income tops $109,000, or $218,000 for a married couple filing jointly.
- The surcharge is added to both your Part B premium and your Part D premium, and it's billed on top of whatever your drug plan already charges.
- Social Security uses your 2025 tax return to set your 2027 IRMAA, so a one-time income spike two years ago can still affect you now.
- If your income has dropped because of a life event like retirement or the death of a spouse, you can ask Social Security to reconsider using Form SSA-44.
What IRMAA actually is
IRMAA isn't a separate bill or a penalty — it's an add-on to your regular Medicare premiums. Most people pay the standard Part B premium and whatever their Part D plan charges. But if your income is higher than the threshold, Medicare tacks on an extra amount for each part. The higher your income, the higher the surcharge.
The key thing to understand: IRMAA looks backward. Your 2027 surcharge is based on the tax return you filed for 2025. That's why a big one-year event — selling a house, cashing out an IRA, taking a large capital gain — can show up as a surprise two years later.
The 2027 income thresholds
For 2026, IRMAA starts above $109,000 in modified adjusted gross income (MAGI) for a single filer, and above $218,000 for a married couple filing jointly. From there, the surcharge climbs in tiers. Each tier adds a set dollar amount to both your Part B and your Part D premium.
There are five income brackets above the base level. The highest tier applies to single filers with MAGI at or above $500,000 and joint filers at or above $750,000. Married people who file separately have their own, tighter set of thresholds — worth checking directly on medicare.gov if that's your situation.
How you'll find out
Social Security mails an IRMAA determination letter, usually in late fall, telling you what you'll owe for the coming year. If you're already collecting Social Security benefits, the Part B surcharge comes out of your monthly check. The Part D surcharge is billed separately — it does not go to your drug plan, even though it's tied to Part D coverage.
That last point trips people up every year. You still owe your plan's regular premium; the IRMAA piece goes to Medicare.
If your income has dropped
You are not stuck with an IRMAA based on outdated income. If you've had a qualifying "life-changing event" — retirement, reduced work hours, marriage, divorce, the death of a spouse, loss of pension income, or a few other situations — you can file Form SSA-44 to ask Social Security to use more recent numbers.
Bring documentation. A retirement date, a benefits statement, or an estate document usually does the job. The form is short, but the difference in what you pay can be significant.
How this fits with the rest of 2027 Medicare
A few 2027 figures are worth knowing while you're planning:
- The Part D out-of-pocket cap is $2,100 in 2026 and $2,400 in 2027.
- The maximum Part D deductible is $615 for 2026 and $700 for 2027.
- The old "donut hole" coverage gap is gone; Part D now has three phases.
- The Medicare Prescription Payment Plan lets you spread drug costs across the year in monthly installments instead of paying a big bill at the pharmacy counter.
Meridian is in Ada County, where Medicare Advantage and stand-alone Part D options are set at the county level. If you're comparing coverage during the Annual Enrollment Period — October 15 through December 7 — the plan finder at medicare.gov shows what's available at your ZIP code.
Common mistakes to avoid
Two errors come up over and over. First, ignoring the IRMAA letter because it "looks like junk mail." Read it carefully; the appeal window is limited. Second, assuming IRMAA is permanent. It's recalculated every year based on the most recent tax data Social Security has, so if your income comes back down, the surcharge should come down with it.

2027 IRMAA income brackets
If your income is above a threshold, you pay an income-related surcharge (IRMAA) on top of your Part B and Part D premiums. Your 2027 surcharge is based on your 2025 tax return. CMS publishes the 2027 brackets and surcharge amounts later this fall, usually in November.
| Income — single filer | Income — married, joint | Part B monthly premium | Part D surcharge |
|---|---|---|---|
| $109,000 or less | $218,000 or less | $202.90 | Plan premium only |
| $109,001 – $137,000 | $218,001 – $274,000 | $284.10 | + $14.50 |
| $137,001 – $171,000 | $274,001 – $342,000 | $405.80 | + $37.40 |
| $171,001 – $205,000 | $342,001 – $410,000 | $527.50 | + $60.40 |
| $205,001 – $500,000 | $410,001 – $750,000 | $649.30 | + $83.30 |
| Above $500,000 | Above $750,000 | $689.90 | + $91.00 |
