Medicare Part D
2027 Medicare IRMAA for Winnemucca, Nev.
WINNEMUCCA, Nev. — If your income has climbed in the past couple of years, your 2026 Medicare bill could look a little different than your neighbor's. Higher earners pay an extra amount on top of the standard Part B and Part D premiums, called the Income-Related Monthly Adjustment Amount, or IRMAA. For 2026, those surcharges kick in once your income crosses $109,000 as a single filer (or $218,000 for a married couple filing jointly), according to the Centers for Medicare & Medicaid Services.

- For 2026, extra Medicare charges called IRMAA start once your income tops $109,000 for single filers or $218,000 for joint filers.
- IRMAA is an add-on to your Part B and Part D premiums, not a separate bill or penalty, and it's based on your tax return from two years earlier (2025 for 2027).
- If a life event like retirement, divorce, or a spouse's death lowered your income, you can file Form SSA-44 to ask Social Security to recalculate your IRMAA using current income.
- IRMAA applies the same way whether you have Original Medicare or Medicare Advantage, and it can be appealed or adjusted each year as your income changes.
Here's what Winnemucca readers should know before the Annual Enrollment Period closes on Dec. 7.
What IRMAA actually is
IRMAA is not a separate bill or a penalty. It's an add-on to your Part B premium (which covers doctor visits and outpatient care) and to your Part D premium (which covers prescription drugs) if your income is above a set threshold.
Most people on Medicare pay the standard Part B premium and no Part D surcharge. Only the roughly top slice of enrollees by income pay IRMAA. If you do, Social Security will send you a letter explaining the amount and how it was calculated.
How Social Security decides
The Social Security Administration looks at your modified adjusted gross income (MAGI) from your tax return two years earlier. So your 2027 IRMAA is based on your 2025 tax return.
That two-year lookback catches a lot of people off guard — especially anyone who sold a home, cashed out an investment, took a large IRA distribution, or retired mid-year in 2025. A one-time income spike can push you into a higher bracket for a single year.
The 2026 income thresholds
IRMAA is tiered. The higher your income, the higher the surcharge. CMS publishes the 2027 brackets later this fall, usually in November, along with the Part B figures.
- Single filer: more than $109,000
- Married filing jointly: more than $218,000
- Married filing separately: the thresholds are much lower — check medicare.gov for the exact figure
From there, the brackets step up several times, with the highest surcharge applying to single filers with incomes at or above roughly $500,000 and joint filers at or above roughly $750,000. Both Part B and Part D have their own surcharge amounts at each tier.
The current dollar amounts for each bracket are published by CMS on Medicare.gov under "Costs."
What's new in 2027 to keep in mind
Even if IRMAA doesn't touch you, a few other figures matter for everyone with Medicare:
- Part D out-of-pocket cap: $2,100 in 2026 and $2,400 in 2027. Once you hit that on covered drugs, you pay nothing more for covered drugs the rest of the year.
- Part D maximum deductible: $615 in 2026 and $700 in 2027.
- The coverage gap ("donut hole") is gone as of 2025. Part D now has three phases instead of four.
- Medicare Prescription Payment Plan: an optional program that lets you spread your drug costs across monthly payments instead of paying a big amount at the pharmacy counter.
A Winnemucca snapshot
Winnemucca sits in Humboldt County, where Medicare beneficiaries choose between Original Medicare (Parts A and B, often paired with a standalone Part D drug plan and sometimes a Medigap policy) and Medicare Advantage. Rural counties in northern Nevada typically have fewer Medicare Advantage options than Reno or Las Vegas, so many local retirees stay with Original Medicare plus Part D — which means the Part D IRMAA surcharge is the one that most often surprises them.
Whichever path you choose, IRMAA is calculated the same way. It follows your income, not your ZIP code.
What to do if your income has dropped
If you had a life-changing event — retirement, the death of a spouse, divorce, loss of pension income, or a work stoppage — you can ask Social Security to recalculate your IRMAA using your current income instead of your 2024 return. This is done by filing Form SSA-44 with documentation.
A big one-time capital gain does not usually qualify on its own, but retirement or a reduction in work hours often does. It's worth the paperwork.
Short FAQ
Q: I just got a letter saying I owe IRMAA for 2027. Can I appeal? Yes. You can request a "New Initial Determination" from Social Security if you had a qualifying life-changing event, or file a formal appeal if you think the income figure is wrong.
Q: Does IRMAA apply if I have Medicare Advantage? Yes. IRMAA is tied to your Part B and Part D enrollment, not to whether you get benefits through Original Medicare or a Medicare Advantage plan.
Q: Is IRMAA permanent? No. It's recalculated every year based on your most recent tax return on file. If your income drops, your surcharge can drop the following year.
Q: How is IRMAA paid? For most people, it's deducted from their Social Security check along with the standard Part B premium. If you don't yet collect Social Security, you'll get a bill from Medicare.

Higher-income surcharges
2026 IRMAA income brackets
If your income is above a threshold, you pay an income-related surcharge (IRMAA) on top of your Part B and Part D premiums. Your 2027 surcharge is based on your 2025 tax return. CMS publishes the 2027 brackets and Part B figures later this fall.
| Income — single filer | Income — married, joint | Part B monthly premium | Part D surcharge |
|---|---|---|---|
| $109,000 or less | $218,000 or less | $202.90 | Plan premium only |
| $109,001 – $137,000 | $218,001 – $274,000 | $284.10 | + $14.50 |
| $137,001 – $171,000 | $274,001 – $342,000 | $405.80 | + $37.40 |
| $171,001 – $205,000 | $342,001 – $410,000 | $527.50 | + $60.40 |
| $205,001 – $500,000 | $410,001 – $750,000 | $649.30 | + $83.30 |
| Above $500,000 | Above $750,000 | $689.90 | + $91.00 |
Standard Part B premium is $202.90/month. Part D surcharges are added to your drug plan’s premium. Married-filing-separately uses a different, compressed schedule. Confirm current amounts at Medicare.gov.
