What IRMAA actually is
IRMAA is not a separate bill you shop for. It is an extra amount the federal government adds to your Part B premium, and to your Part D premium if you have drug coverage, when your income is above certain thresholds. Everyone on Medicare pays the standard Part B premium; higher earners pay that plus a surcharge. The surcharge grows in steps as income rises through five brackets.
Because Medicare Advantage plans include Part B, the Part B IRMAA still applies even if you are enrolled in an Advantage plan. Part D IRMAA applies whether your drug coverage comes through a stand-alone Part D plan or is built into a Medicare Advantage plan with drugs.
The income thresholds for 2027
Social Security looks at your Modified Adjusted Gross Income (MAGI) from two years earlier. For 2027 premiums, that means your 2025 tax return.
For 2026, IRMAA begins above these MAGI levels:
- Single filer: above $109,000
- Married filing jointly: above $218,000
- Married filing separately (lived with spouse): the surcharge structure starts at a much lower threshold and jumps quickly

IRMAA is separate from the plan-level changes taking effect in 2027. Two program-wide facts are worth keeping in view as you compare drug coverage this fall:
- The Part D out-of-pocket cap is $2,100 in 2026 and $2,400 in 2027. Once you hit that on covered drugs, you pay nothing more for covered drugs for the rest of the calendar year.
- The maximum Part D deductible is $615 for 2026 and $700 for 2027, and the old "donut hole" coverage gap is gone — Part D now has three phases.
- The Medicare Prescription Payment Plan lets you spread your drug costs across the year in monthly installments instead of paying big amounts at the pharmacy counter.
