Medicare Part D
2027 IRMAA Thresholds for Medicare in Warwick, R.I.
WARWICK, R.I. — As Medicare's fall Annual Enrollment Period runs through Dec. 7, higher-income retirees are getting a clearer picture of what they'll pay next year. In 2026, an extra charge called IRMAA — the Income-Related Monthly Adjustment Amount — kicks in for single filers with income above $109,000 and joint filers above $218,000. If that's you, the surcharge is added on top of your standard Part B and Part D premiums.

- In 2026, IRMAA is an extra charge added to Part B and Part D premiums for single filers earning above $109,000 and joint filers above $218,000.
- Social Security bases your IRMAA on your income from two years earlier, and most people with Medicare don't pay it at all.
- Going even one dollar over an income bracket puts you in the higher tier for the whole year, though a one-time income spike can be appealed with Form SSA-44 if you've had a qualifying life-changing event.
- Social Security usually mails a predetermination letter in late fall explaining the surcharge, and it's deducted from your benefit check or billed separately if you don't collect Social Security yet.
Higher-income Medicare surcharges for 2027: how the IRMAA brackets work
Here's a plain-English look at how it works, who it affects, and what to check before the year ends.
What IRMAA actually is
IRMAA isn't a separate bill for a separate service. It's an add-on to your monthly Medicare Part B (doctor visits, outpatient care) and Part D (prescription drug) premiums when your income is above certain thresholds. Social Security calculates it automatically using your tax return from two years ago — so your 2027 IRMAA will be based on your 2025 income.
Most people with Medicare do not pay IRMAA. It applies only to the higher-income slice of beneficiaries.
The IRMAA income thresholds
IRMAA uses your Modified Adjusted Gross Income (MAGI) and applies in tiers. For 2026, the surcharge begins above these income levels:
- Single filer: more than $109,000
- Married filing jointly: more than $218,000
- Married filing separately (and lived with spouse): a much lower threshold that begins around the same $109,000 line, with a steep jump above it
From there, the surcharge climbs in five brackets, reaching its highest amount for single filers above roughly $500,000 and joint filers above $750,000. The exact dollar surcharge for each bracket is published by CMS on medicare.gov each year.
Two things worth knowing:
1. It's a cliff, not a phase-in. Earning one dollar over a bracket line puts you in the higher tier for the whole year. 2. IRMAA is recalculated every year based on your most recent tax return on file, so a one-time income bump (like selling a house) can raise your premium for a single year and then fall off.
How you'll be notified
If IRMAA applies to you, Social Security will mail a predetermination letter, usually in late fall, explaining the amount and how it was calculated. If you already receive Social Security, the extra amount is deducted directly from your benefit. If you don't, you'll get a bill from Medicare.
Warwick is in Kent County, and Rhode Island beneficiaries pay IRMAA the same way as everyone else in the country — the thresholds are federal, not state-based.
When you can appeal
You can ask Social Security to reconsider your IRMAA if you've had a life-changing event that reduced your income. Qualifying events include:
- Marriage, divorce, or the death of a spouse
- You or your spouse stopped working or reduced hours
- Loss of income-producing property (not due to sale)
- Loss or reduction of a pension
- An employer settlement payment
You file Form SSA-44 with documentation. If you simply disagree with the calculation — for example, your tax return was amended — you can request a reconsideration through Social Security as well.
Other Part D numbers worth knowing
While you're reviewing coverage this fall, keep these program-wide 2026 figures in mind:
- Part D out-of-pocket cap: $2,100 in 2026 ($2,400 in 2027). Once your out-of-pocket drug costs hit that amount, you pay $0 for covered drugs the rest of the year.
- Part D maximum deductible: $615 in 2026 ($700 in 2027).
- Coverage gap ("donut hole"): gone. Part D now has three phases instead of four.
- Medicare Prescription Payment Plan: an optional program that lets you spread your drug costs across the year in monthly installments instead of paying big amounts at the pharmacy counter.
These changes affect everyone with Part D, IRMAA or not.
Key dates
- Annual Enrollment Period: Oct. 15 – Dec. 7, 2026 (for 2027 coverage)
- Medicare Advantage Open Enrollment: Jan. 1 – March 31, 2027
If you're going to owe IRMAA in 2027, AEP is a good time to make sure your Part D or Medicare Advantage plan still fits — the surcharge is added to whichever Part D plan you choose, so plan quality still matters. The 2027 plan lineup for your area is spelled out in each plan's Annual Notice of Change (ANOC) and on Medicare.gov's Plan Finder.
