Short answer

High-deductible Medigap Plan G is a version of Medigap Plan G, sold in some states, where you pay up to $2,950 in 2026 for Medicare-covered costs before the Medigap policy pays anything.

With high-deductible Medigap Plan G, you must pay the first $2,950 in 2026 of deductibles, copayments and coinsurance for covered services not paid by Medicare, and you pay a separate $250 yearly deductible for foreign travel emergency care.

How does high-deductible Plan G work?

High-deductible Plan G is a high-deductible option that Medicare.gov says Plans F and G offer in some states: you must pay up to $2,950 in 2026 for Medicare-covered costs (coinsurance, copayments and deductibles) before your policy pays anything.

If you have a Medigap policy and get care, Medicare pays its share of the Medicare-approved amount for covered health care costs; the high deductible applies to the costs for covered services that Medicare doesn't pay.

What counts toward the high deductible?

Deductibles, copayments and coinsurance for covered services not paid by Medicare count toward the high deductible. Medicare's guide to Medigap says that if you buy Plan F or Plan G with a high-deductible option, you must pay the first $2,950 in 2026 of deductibles, copayments and coinsurance for covered services not paid by Medicare before the Medigap policy pays anything.

Foreign travel emergency care isn't part of that amount: with a high-deductible option, you pay a separate deductible of $250 a year for it.

How is it different from standard Plan G?

High-deductible Plan G pays nothing until you've paid up to $2,950 in 2026 for Medicare-covered costs; standard Plan G has no such deductible, though it doesn't cover the Part B deductible. Standard Plan G pays 100% of the Part A deductible, Part A and Part B coinsurance or copayments, Part B excess charges, the first 3 pints of blood, hospice coinsurance and skilled nursing facility coinsurance, plus 80% of foreign travel emergency care up to plan limits.

Whether an insurer offers a high-deductible option is one of the things Medicare.gov lists as affecting what a Medigap policy costs. Premiums vary by insurance company, plan and where you live, so compare the same plan from different companies. See how Plan G compares with Plan F.

Is there a high-deductible Plan F?

Yes, in some states, with the same deductible of up to $2,950 in 2026, but only for people who were eligible for Medicare before January 1, 2020. People new to Medicare on or after that date can't buy Plan F in any form; they can buy Plan G instead.

Some people also have an older Plan J with a high-deductible option, bought before January 1, 2006. Medicare.gov says those policies use the same deductible rule.

When can I buy high-deductible Plan G?

Your 6-month Medigap Open Enrollment Period starts the first month you have Part B and are 65 or older. During it, an insurance company can't refuse to sell you any Medigap policy it offers or charge you more because of pre-existing health problems. After it, you may pay more or be turned down, unless you have a guaranteed issue right.

You must generally have Part A and Part B to buy Medigap, and it works only with Original Medicare, not with a Medicare Advantage plan. Medigap plans sold after 2005 don't include drug coverage, so you'd add a separate Medicare drug plan (Part D).

More questions people ask

What is the high deductible for Medigap Plan G in 2026?
Medicare.gov lists it as $2,950 in 2026: you pay up to that amount for Medicare-covered costs before the high-deductible Medigap policy pays anything.
Does high-deductible Plan G cover emergencies abroad?
Yes. After a separate $250 yearly deductible, it pays 80% of billed charges for certain medically necessary emergency care outside the U.S., up to plan limits; Medicare.gov describes the benefit as having a lifetime limit of $50,000.
Can I use high-deductible Plan G with Medicare Advantage?
No. Medigap works only with Original Medicare, and you can't use it to pay Medicare Advantage copayments, deductibles or premiums.
Is high-deductible Plan G sold everywhere?
No. Medicare.gov says the high-deductible version is offered in some states, and insurance companies don't have to offer every Medigap plan. Massachusetts, Minnesota and Wisconsin standardize Medigap differently.
