GLENDALE, Ariz. — Fall 2026. If you live with a long-term health condition like diabetes, heart disease, chronic lung disease or end-stage renal disease, there is a specific slice of Medicare Advantage built for you: Chronic Condition Special Needs Plans, or C-SNPs. For 2027, Maricopa County — the county Glendale sits in — has 25 C-SNPs on the Medicare Advantage menu, offered by 11 carriers. A few are new, a few have been renamed, and all of them require a qualifying diagnosis confirmed by a provider before you can enroll.

Here is what that means for your choices during the Annual Enrollment Period, which runs October 15 through December 7, 2026, for coverage that starts January 1, 2027.

What is a C-SNP, and how does it work in Glendale for 2027?

A C-SNP is a type of Medicare Advantage plan designed around a specific chronic condition. In Maricopa County for 2027, 25 of the 92 Medicare Advantage plans are C-SNPs. (Two other Special Needs Plan types also exist locally: 10 Dual Eligible SNPs for people with both Medicare and Medicaid, and 5 Institutional SNPs for people in long-term care settings.)

Like other Medicare Advantage plans, a C-SNP replaces the way Original Medicare pays your Part A and Part B claims, and almost all of them include Part D drug coverage. What makes a C-SNP different is that its network, drug list and care management are shaped around the condition it serves. You generally see in-network providers, and the plan coordinates care tied to that diagnosis.

Because these plans only accept members with the condition they are built for, you cannot simply pick one off the shelf. Enrollment depends on your diagnosis.

Who qualifies for a C-SNP in Maricopa County?

To join a C-SNP, you must have the qualifying chronic condition the plan is built for, and a provider has to confirm that diagnosis. Medicare recognizes 15 chronic conditions that can anchor a C-SNP, including diabetes, cardiovascular disorders, chronic heart failure, chronic lung disorders and end-stage renal disease.

When you apply, the plan sends a short form to your doctor's office to verify the diagnosis. If your provider confirms it, your enrollment moves forward. If the plan cannot confirm the condition within the window Medicare sets, the enrollment will not take effect.

You also need to be enrolled in Medicare Part A and Part B and live in the plan's service area, which for the plans in the table below means Maricopa County.

Which carriers offer C-SNPs in Maricopa County for 2027?

Eleven carriers offer C-SNPs in Maricopa County for 2027. In alphabetical order:

The table below lists every C-SNP by carrier and plan type. Several are new for 2027, including Aetna Medicare Chronic Care, DEVOTED C-SNP ENHANCED 092 AZ, DEVOTED C-SNP GIVEBACK EXTRAS 104 AZ, DEVOTED C-SNP PLUS 093 AZ, Gold Dialysis Premier, SCAN DaVita Dialysis Care Complete and SCAN Walmart Medicare Complete. Three Alignment plans and one Devoted plan have been renamed; the Annual Notice of Change (ANOC) from each insurer spells out what else is changing.

Important: you can tell which condition a plan serves only when the plan's name states it. For plans whose names do not name a condition, call the carrier or check the plan's Evidence of Coverage before you assume what they cover.

When can I enroll in a C-SNP in Glendale?

The Annual Enrollment Period — October 15 through December 7, 2026 — is the main window. A change you make during that window takes effect January 1, 2027. If you do nothing, your current plan simply renews under the terms in its ANOC.

C-SNPs also come with a Special Enrollment Period tied to your condition. If you are newly diagnosed with a qualifying chronic condition, you can enroll in a matching C-SNP outside the regular window; this SEP generally runs until you enroll in a plan. There is also an SEP if a C-SNP determines you no longer have the qualifying condition, which gives you time to switch to another Medicare Advantage plan or back to Original Medicare.

The Medicare Advantage Open Enrollment Period (January 1 – March 31, 2027) gives Medicare Advantage members one more chance to switch plans or return to Original Medicare, though it is not a general enrollment window for people on Original Medicare.

What else is changing for 2027 that affects C-SNP members?

A couple of program-level numbers are confirmed for 2027. The Part D out-of-pocket cap rises to $2,400 (up from $2,100 in 2026), and the Part D maximum deductible rises to $700 (up from $615 in 2026). Because almost every C-SNP in Maricopa County includes drug coverage, these caps apply to you as a member.

CMS has not yet announced the 2027 Part B standard premium and deductible or the 2027 IRMAA income brackets; those typically come out later in the fall. We will update the figures across MyMedicarePlans.org as soon as they are published.

Across Maricopa County's full 2027 Medicare Advantage lineup, 13 plans are not returning, 17 are new, and 13 have been renamed. If your current plan is one of them, your ANOC will say so — read it when it arrives.

Frequently asked questions

Do I need my doctor to confirm my diagnosis before I can join a C-SNP?
Yes. Enrollment in a C-SNP depends on a qualifying chronic condition verified by a provider. When you apply, the plan sends a form to your doctor's office to confirm the diagnosis. If the condition is not confirmed within the window Medicare sets, the enrollment will not take effect.
I was just diagnosed with a qualifying condition. Do I have to wait for Annual Enrollment to join a C-SNP?
No. A new diagnosis of a qualifying chronic condition triggers a Special Enrollment Period that lets you join a matching C-SNP outside the October 15 – December 7 window. The SEP generally continues until you enroll in a plan.
What happens if my C-SNP decides I no longer have the qualifying condition?
You get a Special Enrollment Period to move to another Medicare Advantage plan or return to Original Medicare (and pick up a Part D plan if you need drug coverage). The plan has to notify you and give you a grace period before coverage ends.
Where can I compare the specific costs and networks of these C-SNPs?
The Medicare Plan Finder at medicare.gov lets you enter your ZIP code and your drugs to compare plans side by side. You can also call 1-800-MEDICARE (TTY 1-877-486-2048), 24 hours a day, 7 days a week, or ask any plan for its Evidence of Coverage and provider directory before you enroll.
