Compared with 2026, Central Health Medicare Plan and Providence Medicare Advantage Plans are not offering plans in the county, and VillageHealth is new to the county.
Alignment Health Plan has the largest single-carrier count in the county with 17 plans, followed by Elevance Health (Anthem) at 12 and SCAN Health Plan at 10. Others, like Providence Medicare Advantage Plans and CalOptima Health OneCare, offer a single plan in the county.
How the plan types differ
The 104 plans fall into three types. Knowing what each type does helps you read the table:
- HMO (75 plans): You usually pick a primary care doctor and stay in the plan's network. Referrals are often needed for specialists.
- HMO-POS (21 plans): Like an HMO, but with a "point of service" option to see certain providers outside the network for some services.
- PPO (8 plans): You can see providers in or out of network, though out-of-network care usually costs more.
None of the 92 plans in Orange County is a stand-alone PPO without drug coverage; every plan in the county's 2027 lineup includes Part D drug coverage built in. Special Needs Plans in the mix
Of the 104 plans, 40 are Special Needs Plans (SNPs). SNPs are Medicare Advantage plans built for people who fit a specific group:
- D-SNP — for people who have both Medicare and Medicaid.
- C-SNP — for people with certain chronic conditions, like diabetes or heart disease.
- I-SNP — for people who live in a nursing home or need that level of care at home.
You have to qualify to enroll in one. The table marks each SNP with its type in parentheses so you can see at a glance which plans are general-market and which are for specific groups.
What to check before you enroll
Availability is just the first filter. Once you know a plan is offered in the county, look at:
- Whether your doctors and hospitals are in the plan's network.
- Whether your prescriptions are on the plan's formulary and at what tier.
- The plan's monthly premium (which is on top of your Part B premium), deductible, and copays.
- The plan's Star Rating, which CMS updates each year.
For 2027, some program-wide Part D rules apply no matter which plan you pick: the Part D out-of-pocket cap is $2,400, the maximum Part D deductible is $700, and the old "donut hole" coverage gap is gone (it was eliminated in 2025). If your drug costs are uneven month to month, the Medicare Prescription Payment Plan lets you spread them across the year.
Higher-income enrollees should also know that IRMAA surcharges on Part B and Part D start above $109,000 in income for a single filer in 2026.

- Oct. 15 – Dec. 7: Medicare Annual Enrollment Period. You can join, switch, or drop a Medicare Advantage or Part D plan for coverage starting Jan. 1.
- Jan. 1 – March 31: Medicare Advantage Open Enrollment Period. If you're already in a Medicare Advantage plan, you can switch to another MA plan or go back to Original Medicare once during this window.
