What a C-SNP is and who qualifies

A Chronic Condition Special Needs Plan is a Medicare Advantage plan designed for people with a specific, severe or disabling chronic condition recognized by Medicare. Enrollment is not open to everyone — a health care provider has to verify that the person has the qualifying condition the plan is built to serve. If that verification doesn't come through within the window the plan allows, enrollment can't continue.

C-SNPs are not a separate program from Medicare Advantage; they are a category within it. Like other Medicare Advantage plans, each C-SNP has its own service area, its own network, its own drug formulary and its own cost-sharing. Those details are not included here. They live on the Medicare Plan Finder at Medicare.gov, in each plan's Evidence of Coverage, and in the plan's provider and pharmacy directories.

The statewide count and the carriers offering them

For 2027, the CMS Landscape file lists 77 C-SNPs available somewhere in California. The other Special Needs Plan categories in the state are 50 Dual Eligible Special Needs Plans (D-SNPs) and 8 Institutional Special Needs Plans (I-SNPs). All three sit inside the 394-plan Medicare Advantage total.

Thirteen carriers are offering C-SNPs in California next year. In alphabetical order, with the number of C-SNPs each one has in the statewide file: Aetna (CVS Health) (5), AHF (1), Alignment Health Plan (11), Astiva Health (5), Centene (WellCare) (2), Champion Heath Plan (8), Clever Care Health Plan (4), Community Health Group (1), Elevance Health (Anthem) (12), Imperial Health Plan of California, Inc. (2), SCAN Health Plan (13), UnitedHealthcare (11), and VillageHealth (2).

The conditions named in plan names, only as named

Many C-SNP names don't spell out the condition. Some do, and those are the only ones we can describe by condition here.

Within Alignment Health Plan's lineup, several plan names include "Heart & Diabetes" — Alignment Health Heart & Diabetes 010, 048, 051 and 054, along with Alignment Health Heart & Diabetes Plus 039, 044 and 045. Alignment Health BreathEasy Plus 041 names a breathing-related focus in its name.

Within Elevance Health (Anthem)'s CareMore-branded line, several plan names call out the condition: Anthem I CareMore Kidney Care (listed twice at the plan level), Anthem I CareMore Lung Care and Anthem I CareMore Lung Care 2. Anthem Chronic Care Advantage and Anthem Kidney Care appear in the PPO C-SNP column, with "Kidney Care" named in the second.

From VillageHealth, SCAN DaVita Dialysis Care and SCAN DaVita Dialysis Care Complete name dialysis in their plan names.

Other plan names in the state file — for example, Aetna Medicare Chronic Care, PHP, Astiva Health's C-SNP line, the Wellcare Specialty Simple plans, the Champion lineup, the Clever Care plans, Community y Más, Imperial Flex, Imperial Senior Value, SCAN Ansim Care, SCAN Balance, SCAN Strive, and the UHC Complete Care and UHC Complete Care Support plans — do not state a condition in their names, so we aren't attaching one here. The specific condition each one serves is listed in the plan's own materials and on Medicare.gov.

The Special Enrollment Period for people with a qualifying condition

The Annual Enrollment Period runs October 15 – December 7, 2026, and a change made during it takes effect January 1, 2027. For people who qualify for a C-SNP, Medicare also offers a Special Enrollment Period tied to the chronic condition itself. Once a provider confirms the qualifying condition, that SEP generally allows a one-time enrollment into a C-SNP outside the regular AEP window.
