The Centers for Medicare & Medicaid Services (CMS) has released the 2027 plan landscape, and Riverside County — which includes Moreno Valley — will see 17 plans leave after 2026, 14 new plans arrive, and 20 plans keep running under a new name. Another 32 continuing plans will differ from their 2026 version on premium, deductible, plan type or name, and 54 are continuing with no listed change.

Plans leaving after 2026
Seventeen plans will not be offered in Riverside County in 2027. If your plan is on this list, your coverage under it ends Dec. 31, 2026, and you will need to choose new coverage.
The non-renewing plans are: Alignment Health Honor+ Plan (HMO), Alignment Health My Choice Select (HMO), Alignment Health the ONE (HMO), Astiva Health Premier Plan (HMO), Wellcare Giveback (HMO), Central Health Classic Care Plan I (HMO), Central Health Embrace Care Plan (HMO C-SNP), Central Health Embrace Choice Plan (HMO C-SNP), Central Health Medicare Plan (HMO), Central Health Part B Savings Plan (HMO), Central Health Valor Care Plan (HMO), Champion Select (HMO-POS C-SNP), Humana Gold Plus H5619-148 (HMO), Kaiser Permanente Dual Complete South P1 (HMO D-SNP), Molina Medicare Choice Care (HMO), SCAN Affirm partnered with Included LGBTQ+ Health (HMO), and SCAN MyChoice (HMO).
Central Health Medicare Plan is the one carrier fully exiting the county. Its members will need to move to another carrier for 2027.
New plans for 2027
Fourteen plans are entering the Riverside County market for 2027: Aetna Medicare Chronic Care (HMO C-SNP), Alignment Health Honor Plus 060 (HMO), Astiva Health C-SNP Bloom (HMO C-SNP), Astiva Health C-SNP Fountain (HMO C-SNP), Champion Compass (HMO C-SNP), Champion Home (HMO C-SNP), Champion Secure (HMO C-SNP), Clever Care Breathe Select+ (HMO C-SNP), Clever Care Total Choice+ (HMO C-SNP), Humana Gold Plus H5619-191 (HMO), Imperial Flex (HMO C-SNP), SCAN Costco Medicare Advantage (HMO), SCAN Inclusive (HMO), and SCAN DaVita Dialysis Care (HMO-POS C-SNP).
Many of the new options are Chronic Condition Special Needs Plans (C-SNPs), which are built around specific conditions like diabetes, heart disease or lung disease. You only qualify for a C-SNP if you have the condition it covers.
Renamed plans — same plan, new label
Twenty plans are being renamed for 2027. The plan may look "new" in the mail even though it is the continuation of what you have today. Examples include several Aetna Medicare Prime and Signature plans, a long list of Alignment Health C-SNPs, Wellcare Health Net Dual Liberty (previously Wellcare Dual Liberty), two AARP Medicare Advantage from UnitedHealthcare plans, and SCAN DaVita Dialysis Care Complete (previously listed as VillageHealth).
A rename by itself does not force you to do anything. But CMS also flags 32 continuing plans as differing on premium, deductible, plan type or name compared to 2026 — so the safest move is still to read the ANOC. That is the letter your plan is required to send by Sept. 30 that spells out exactly what is changing.
What to do if your plan is leaving
If your Medicare Advantage plan is not renewing for 2027, CMS gives you a Special Enrollment Period (SEP) that runs from Dec. 8, 2026, through Feb. 28, 2027. During that window you can:
- Enroll in a different Medicare Advantage plan offered in Riverside County, or
- Return to Original Medicare (Parts A and B) and add a standalone Part D drug plan, and, if eligible, apply for a Medicare Supplement (Medigap) policy.
You can also make a change during the Annual Enrollment Period (AEP), Oct. 15 – Dec. 7, which is open to everyone with Medicare. If you enroll in a new Medicare Advantage plan during AEP, that coverage starts Jan. 1, 2027. And from Jan. 1 through March 31, the Medicare Advantage Open Enrollment Period lets anyone already in a Medicare Advantage plan make one switch — either to another Medicare Advantage plan or back to Original Medicare.

- Part D out-of-pocket cap: $2,100 in 2026.
- Part D maximum deductible: $615.
- The old coverage gap ("donut hole") is gone as of 2025 — Part D now has three phases.
- The Medicare Prescription Payment Plan is available if you want to spread drug costs over the year in monthly amounts instead of paying at the pharmacy counter.
- IRMAA income-related surcharges for Part B and Part D start above $109,000 for single filers in 2026.
