What's on the menu in Ontario for 2026
Medicare Advantage availability is set at the county level by the Centers for Medicare & Medicaid Services (CMS). Because Ontario is in San Bernardino County, the plans available to someone with a San Bernardino County ZIP code are the plans in the list below.
The 2026 county roster includes national names like Aetna (CVS Health), Humana, UnitedHealthcare, Elevance Health (Anthem) and Kaiser Permanente, along with California-focused carriers such as Blue Shield of California, SCAN Health Plan, Alignment Health Plan, Central Health Medicare Plan and Molina Healthcare of California. Smaller and specialty carriers round out the list: Astiva Health, Centene (WellCare), Champion Heath Plan, Clever Care Health Plan, Elite Health Plan, IEHP DualChoice, Imperial Health Plan of California and VillageHealth.
How the plan types differ
Every plan in the county roster is one of three types, and the type shapes how you use the plan.
- HMO (Health Maintenance Organization): You generally choose a primary care doctor and use the plan's network. Out-of-network care is usually not covered except in emergencies. 69 of the 98 plans are HMOs.
- HMO-POS (Point of Service): An HMO with a limited option to see certain out-of-network providers for some services. 25 plans use this design.
- PPO (Preferred Provider Organization): More flexibility to see out-of-network providers, usually at a higher cost. Only 4 PPO plans are on the 2026 list for the county.
None of the plans listed for San Bernardino County are Private Fee-for-Service (PFFS) or Medical Savings Account (MSA) plans this year.
Special Needs Plans are a big part of the local list
Of the 98 plans available, 40 are Special Needs Plans, or SNPs. SNPs are Medicare Advantage plans limited to people who meet specific criteria:

Ontario's roster includes examples of all three: chronic-condition plans from carriers like Alignment Health Plan, Champion Heath Plan, Clever Care Health Plan and SCAN; dual-eligible plans such as IEHP DualChoice, Wellcare Health Net Dual Liberty, Kaiser Permanente Dual Complete South and Molina Medicare Complete Care Plus; and institutional plans such as SCAN Embrace and SCAN Embrace Together. You can only enroll in an SNP if you meet its rules.
Drug coverage is built into every plan on the list
All 98 Medicare Advantage plans offered in San Bernardino County for 2026 include Part D prescription drug coverage.
