RIVERSIDE, Calif. — As the Medicare Annual Enrollment Period opens for 2027 coverage, people in Riverside who live with a serious long-term illness have a specific lane to consider: Chronic Condition Special Needs Plans, known as C-SNPs. These are Medicare Advantage plans built around one qualifying diagnosis, and the lineup in Riverside County shifts next year with new plans joining and several familiar ones carrying new names.

For plan year 2027, Riverside County residents can choose from 33 C-SNPs offered by 10 carriers, according to the CMS Medicare Advantage Landscape Source File. The window to enroll, switch or drop a plan runs October 15 through December 7, 2026, with changes taking effect January 1, 2027.

What is a C-SNP and how does it work in Riverside County?

A C-SNP is a type of Medicare Advantage plan that only enrolls people living with a specific severe or disabling chronic condition. Riverside is in Riverside County, where every C-SNP available for 2027 includes Part D drug coverage and is designed around care for one qualifying diagnosis.

Medicare lists 15 conditions that can qualify a plan as a C-SNP, including diabetes, chronic heart failure, cardiovascular disorders, chronic lung disorders, end-stage renal disease requiring dialysis, and certain neurologic conditions. If verification does not come through within the time Medicare allows, enrollment can be cancelled.

You also need to have Medicare Part A and Part B and live in the plan's service area. Nothing about being on a C-SNP changes your underlying Medicare — you are still in the Medicare program; you are simply getting your Part A, Part B and Part D benefits through a specialized Medicare Advantage plan.

When can someone with a qualifying condition sign up?

People with a qualifying chronic condition get a Special Enrollment Period to join a C-SNP, so they are not limited to the October 15–December 7 Annual Enrollment Period. This SEP lets you enroll in a C-SNP once at any time of year after you are diagnosed, as long as a plan serving your condition is offered where you live.

Once enrolled in a C-SNP, the usual Medicare Advantage rules apply for future changes. The Medicare Advantage Open Enrollment Period (January 1 – March 31) also allows a one-time switch for current Advantage members. If the chronic condition is later found to no longer apply, the plan must give a grace period before disenrolling the member.

Several plans are new for 2027, including Aetna Medicare Chronic Care, Astiva Health C-SNP Bloom, Astiva Health C-SNP Fountain, Champion Compass, Champion Home, Champion Secure, Clever Care Breathe Select+, Clever Care Total Choice+, Imperial Flex, and SCAN DaVita Dialysis Care. Alignment Health Plan has renamed all seven of its C-SNPs for 2027, and VillageHealth has renamed its dialysis-focused plan to SCAN DaVita Dialysis Care Complete. A rename does not automatically mean the benefits are the same — the Annual Notice of Change (ANOC) from the current plan spells out what is different.

The 2027 Part B standard premium and the 2027 IRMAA income brackets have not yet been announced; CMS typically publishes them later in the fall.

What should current C-SNP members in Riverside do this fall?

If you are already in a C-SNP, read the Annual Notice of Change your plan mailed in late September. It explains what is changing for 2027 — premium, drug list, provider network, cost sharing and plan name. If your current plan is being renamed or restructured, the ANOC is where those details live.
