What's changing in the county lineup

Thirteen carriers currently sell Medicare Advantage plans in San Francisco County. For 2027, Central Health Medicare Plan is not returning. The other 12 carriers — Aetna (CVS Health), Align Senior Care, Alignment Health Plan, CCHP (Chinese Community Health Plan), Centene (WellCare), Elevance Health (Anthem), Humana, Imperial Health Plan of California, Kaiser Permanente, San Francisco Health Plan, SCAN Health Plan and UnitedHealthcare — are still offering plans here, though several are changing which plans they sell or what those plans are called.

Across the county, CMS data show 22 plans with at least one change on name, type, premium or deductible, and 26 continuing with no such change. The table below lists every plan available in San Francisco County; this article walks through the moves that matter most this fall.

Plans leaving after 2026

Twelve Medicare Advantage plans currently sold in San Francisco County will not be offered in 2027:

If you are enrolled in any of these plans, you will get a non-renewal notice from your plan. Medicare offers a Special Enrollment Period when your plan leaves your area: you have from Dec. 8, 2026 through Feb. 28, 2027 to pick a new Medicare Advantage plan or return to Original Medicare and add a stand-alone Part D plan. You do not have to wait for a new window — you can also make your choice during the Annual Enrollment Period and start fresh on Jan. 1.

Plans new for 2027

Seven Medicare Advantage plans are new to San Francisco County for 2027:

A C-SNP is a Chronic Condition Special Needs Plan, meant for people with certain qualifying health conditions. A D-SNP is a Dual Eligible Special Needs Plan, for people who have both Medicare and Medi-Cal.

Renames and other changes

Thirteen plans are keeping the same bones but changing names for 2027. The biggest cluster is at Alignment Health Plan, where many C-SNPs are picking up a numbered suffix (for example, the plan previously called Alignment Health Heart & Diabetes (HMO C-SNP) is now Alignment Health Heart & Diabetes 010 (HMO C-SNP)). Centene is adding "Health Net" to its D-SNP name, and the plan that was Alignment Health Harmony (HMO) will be offered as Central Health Harmony 031 (HMO). Aetna Medicare Signature Extra is shifting from an HMO-POS to an HMO.

If a plan you are in has only been renamed, your coverage simply continues under the new name — but you should still read your Annual Notice of Change (ANOC), mailed by late September, to see what else is moving.

What stays the same about the choice itself

Even with the lineup shifting, the basics of how you shop do not change:

Some program-level numbers to keep in mind as you compare drug coverage: in 2026, the Part D out-of-pocket cap is $2,100 and the maximum Part D deductible is $615. For 2027, the out-of-pocket cap rises to $2,400 and the maximum deductible rises to $700. The Medicare Prescription Payment Plan, which lets you spread drug costs across the year in level monthly payments, remains an option.

The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet. CMS typically publishes those figures later in the fall.

You will get a non-renewal letter from the plan. You can choose a new Medicare Advantage plan during the Annual Enrollment Period (Oct. 15 – Dec. 7, 2026) for a Jan. 1 start, or use the Special Enrollment Period that runs Dec. 8, 2026 through Feb. 28, 2027 for people whose plans leave the area. You can also return to Original Medicare and sign up for a stand-alone Part D drug plan during that same window.
Do I need to do anything if my plan is only being renamed?
