Los Angeles Medicare Advantage for 2027: 26 new plans, 23 not returning, one carrier out

People in Los Angeles can choose from 124 Medicare Advantage plans for 2027, against 121 this year. 23 of this year’s plans are gone, 26 are new to Los Angeles County, and 61 that continue have changed their premium, deductible, plan type or name, 23 of them under a new name.

LOS ANGELES — As the Annual Enrollment Period opens on October 15, 2026, Medicare shoppers in Los Angeles will see a reshuffled Medicare Advantage lineup for 2027. Twenty-three plans are leaving the Los Angeles County market after 2026, 26 new plans are joining, and one carrier — Central Health Medicare Plan — is not returning at all. Another 75 plans are continuing, so most people who do nothing will stay on a plan that renews on January 1, 2027.

Los Angeles is in Los Angeles County, where Medicare plan availability is set. The counts and plan names below come from the CMS Medicare Advantage Landscape Source Files for 2026 and 2027. The table on this page lists every plan by carrier.

The 2027 lineup at a glance

For the 2026 plan year, 21 carriers offer 121 Medicare Advantage plans in Los Angeles County. By type, that breaks down to 89 HMO plans, 24 HMO-POS plans and 8 PPO plans. Forty-seven of those plans are Special Needs Plans (for people with certain chronic conditions, dual Medicare-Medicaid eligibility, or who live in an institutional setting), and all 121 include prescription drug coverage.

The 2027 lineup will shift from that starting point. Even without counting new entries, the county is losing one carrier and gaining 26 brand-new plans across others. The table below shows the full roster — carrier by carrier, plan by plan.

Plans not returning for 2027

Twenty-three Medicare Advantage plans in Los Angeles County will not be offered in 2027. Here they are, grouped by carrier (alphabetical):

Central Health Medicare Plan is the only carrier leaving the Los Angeles County market entirely. If you are on one of its plans — or any other non-renewing plan — your insurer will mail a notice before the end of 2026 explaining that coverage ends December 31. You then qualify for a Special Enrollment Period to pick a new Medicare Advantage plan or return to Original Medicare (with the option of adding a stand-alone Part D plan). That SEP generally runs from December 8, 2026 through the end of February 2027, but your notice will spell out the exact dates for your situation.

Plans new for 2027

Twenty-six Medicare Advantage plans are new to Los Angeles County for 2027. By carrier:

A good chunk of the new slate is Chronic Condition Special Needs Plans (C-SNPs), which are built around specific conditions like heart disease, diabetes or lung disease. If you have one of those conditions, it is worth checking the plan's rules — enrollment usually requires a doctor's confirmation of the diagnosis.

Renamed and changed plans

Twenty-three plans will show up under a new name in 2027 but are the same plan continuing from 2026. Many of these are Aetna, Alignment Health, Blue Shield and UnitedHealthcare plans that added or shifted a number or wording in the plan name (for example, Alignment Health My Choice is renamed Alignment Health My Choice 001). If you are already enrolled, the plan ID in your Annual Notice of Change will match your current coverage, even if the name reads a little differently.

Across the county, 38 plans will have at least one change for 2027 to premium, deductible, type or name — the kind of year-to-year adjustment every Medicare Advantage plan can make. The specifics are in the Annual Notice of Change your plan mailed in September, and in the Medicare Plan Finder at medicare.gov once 2027 plan details go live.

What is changing in Medicare itself for 2027

A few program-level numbers are worth knowing as you compare plans:

• Part D out-of-pocket cap: $2,400 in 2027 (up from $2,100 in 2026). Once your drug spending hits that cap, you pay $0 for covered prescriptions the rest of the year.
• Part D maximum deductible: $700 in 2027 (up from $615 in 2026). Many plans set a lower deductible, but none can set it higher.
• Medicare Prescription Payment Plan: Still available in 2027 as a free option to spread your drug costs into monthly bills instead of paying at the pharmacy.
• 2027 Part B premium and deductible, and the 2027 IRMAA income brackets: Not yet announced. CMS publishes these later in the fall, usually in November.

How to use this during the Annual Enrollment Period

Open enrollment runs October 15 through December 7, 2026, and any choice you make takes effect January 1, 2027. If you do nothing and your current plan is on the "continuing" list, it renews automatically with the changes described in its Annual Notice of Change.
