STOCKTON, Calif. — The window to change your Medicare coverage for 2027 is open now and closes Dec. 7, 2026. If you already like your plan, you don't have to do anything — it will roll over into next year with whatever changes the insurer has listed in its Annual Notice of Change. But this is the one time of year when you can switch Medicare Advantage or Part D plans freely, so a short review now is worth the time.
Here is a plain checklist for Stockton-area readers, built around what actually changes from one plan year to the next.
What is the Dec. 7 Medicare deadline for Stockton residents?
Dec. 7, 2026 is the last day to join, switch or drop a Medicare Advantage or Part D plan for 2027 coverage. The Annual Enrollment Period opened Oct. 15, and any choice you make during the window takes effect Jan. 1, 2027.
If you do nothing, your current plan simply renews — assuming the insurer is offering it again next year — with the changes spelled out in its Annual Notice of Change. There is a second, narrower window after that: the Medicare Advantage Open Enrollment Period from Jan. 1 to March 31, 2027, which lets people already in an Advantage plan make one switch.
How do I read my Annual Notice of Change for 2027?
Your ANOC is the single most important document in this review. Insurers mailed (or emailed) it by late September, and it lays out — side by side — what your plan looks like in 2026 versus what it will look like in 2027.
Walk through it with four questions in mind:
- Premium and deductible. Did either go up? For Part D specifically, 2027's maximum allowable deductible is $700.
- Copays and coinsurance for the services you actually use — primary care, specialists, imaging, hospital stays.
- Drug tiers. A medication you take may have moved to a higher tier, or come off the formulary.
- Extras. Dental, vision, hearing, OTC and fitness benefits can change year to year.
If anything in the ANOC looks off, that is your cue to compare other plans before Dec. 7.
How do I check my Stockton doctors, hospitals and prescriptions against next year's plan?
Check three directories separately — a plan can keep your doctor but drop your hospital, or vice versa.
1. Doctors. Call your primary care office and any specialists you see and ask whether they will be in-network with your plan for 2027. "In-network this year" is not a promise about next year. 2. Hospitals. Confirm the Stockton-area hospitals you'd want to use — including the system your doctors admit to — are contracted for 2027. 3. Prescriptions. Pull up the 2027 formulary (your ANOC links to it) and search each drug you take. Note the tier, any prior authorization, and whether your pharmacy is a preferred one.
The fastest way to compare across plans is the Medicare Plan Finder at medicare.gov. Enter your ZIP code and your drug list, and it will show every 2027 plan available in San Joaquin County along with estimated yearly drug costs at your pharmacy.
How many Medicare Advantage plans and carriers are offered in San Joaquin County?
Stockton is in San Joaquin County, where 44 Medicare Advantage plans from 12 carriers are on the 2026 landscape, according to the CMS Medicare Advantage Landscape file. Of those, 34 are HMOs, 4 are HMO-POS, and 6 are PPOs; 19 are Special Needs Plans designed for people with specific chronic conditions or who have both Medicare and Medi-Cal; and 41 include prescription drug coverage.
The carriers offering plans in the county are Aetna (CVS Health), Alignment Health Plan, Blue Shield of California, Centene (WellCare), Central Health Medicare Plan, Elevance Health (Anthem), Health Plan of San Joaquin / Mountain Valley Health Plan Advantage D-SNP, Humana, Imperial Health Plan of California, Kaiser Permanente, SCAN Health Plan and UnitedHealthcare. The table below lists every plan by carrier so you can see what is available alongside what you have today. 2027's lineup publishes on the Plan Finder as the enrollment period runs.
What is changing in Part D drug coverage for 2027?
Two numbers to know going in. In 2027, the Part D out-of-pocket cap moves to $2,400 (it is $2,100 in 2026), and the maximum Part D deductible moves to $700 (it is $615 in 2026). Once you hit the cap, you pay $0 for covered drugs for the rest of the calendar year.
The old "donut hole" is gone — it was eliminated in 2025, and Part D now runs in three phases: deductible, initial coverage, and catastrophic. You can also ask your plan to enroll you in the Medicare Prescription Payment Plan, which spreads your drug costs across monthly payments instead of charging the full amount at the pharmacy counter.
A few 2027 figures are not out yet. CMS announces the Part B standard premium and deductible and the IRMAA income brackets later in the fall, usually in November. Watch medicare.gov for those.
Where can Stockton residents get free, unbiased help?
Three places, all free:
- medicare.gov — the Plan Finder is the official comparison tool and uses current-year plan data.
- 1-800-MEDICARE (1-800-633-4227) — open 24/7, with help in English, Spanish and other languages.
- Your plan's member services line — on the back of your member ID card — for questions about your specific ANOC.
Be cautious with unsolicited calls, texts and door knocks this time of year. Medicare will not call you to sell you a plan.
