Jacksonville is in Duval County, where 24 D-SNPs will be offered next year from seven carriers, with some plans new, some renamed, and the rest returning. The Annual Enrollment Period runs Oct. 15 – Dec. 7, 2026, and any change you make now takes effect Jan. 1, 2027 — but if you qualify as a "dual eligible," you also have more chances to switch than most people.
What a D-SNP is, in plain terms
A Dual Special Needs Plan is a type of Medicare Advantage plan built for people who have Medicare and also get help from Medicaid. Every D-SNP in Duval County includes prescription drug coverage, and each one coordinates with Florida Medicaid in some way — some tightly, some loosely, depending on the plan design.

Others have been renamed — for example, several of UnitedHealthcare's UHC Dual Complete plans carry new plan IDs for 2027, and some Humana D-SNPs now use the "Dual Select" name. How often you can change plans
Most people with Medicare can only change plans during the Annual Enrollment Period (Oct. 15 – Dec. 7) or the Medicare Advantage Open Enrollment Period (Jan. 1 – March 31). Dual-eligible members have more flexibility:
- If you have Medicaid or qualify for Extra Help with Part D, you generally get a monthly Special Enrollment Period to join an integrated D-SNP or to move to Original Medicare with a stand-alone Part D plan.
- You can also change plans when your eligibility status changes — for example, if you gain, lose or change your Medicaid category.
