The three plan types on the list work differently, and that difference matters more than the brand name on the card.
- HMO (Health Maintenance Organization). You use doctors and hospitals in the plan's network, and you usually need a primary care doctor and referrals for specialists.
- HMO-POS (Point of Service). An HMO with a limited option to go out of network for certain services, usually at a higher cost.
- PPO (Preferred Provider Organization). More flexibility to see out-of-network providers, though staying in-network still costs less.
Some listings you will see in the table include a "Regional PPO." That is a PPO with a service area covering a larger multi-county region rather than a single county.
Special Needs Plans, briefly
Nearly half of Duval County's 2027 plans are SNPs. You can only join an SNP if you meet its eligibility rules:

- C-SNP — for people with a qualifying chronic condition, such as diabetes, heart disease, or end-stage renal disease.

- Part D has a $2,100 out-of-pocket cap on covered drugs in 2026. Once you hit it, you pay $0 for covered drugs the rest of the year.
- The maximum Part D deductible a plan can charge is $615.
- The old coverage gap ("donut hole") is gone as of 2025. Part D now has three phases: deductible, initial coverage, and catastrophic.
