JACKSONVILLE, Fla. — Fall 2026. When the Annual Enrollment Period opens Oct. 15, Duval County shoppers will see 15 Medicare Advantage plans on the shelf that were not sold here a year ago. The federal Centers for Medicare & Medicaid Services (CMS) confirmed the 2027 lineup in its Landscape Source File, and the new plans come from 10 different carriers — a mix of standard HMOs and PPOs, Dual Eligible Special Needs Plans (D-SNPs), Chronic Condition SNPs (C-SNPs) and an Institutional SNP (I-SNP).
A "new" plan is not simply a renamed version of something you already have. Special Needs Plans among the new ones
Several of the new Jacksonville plans are Special Needs Plans, which only accept people who meet the eligibility rules:
- D-SNP (Dual Eligible): for people who have both Medicare and Medicaid. Among the new 2027 plans, examples include Wellcare Sunshine Health Dual Access, Wellcare Sunshine Health Dual Align, Simply Complete Platinum, Simply Integrated Platinum, Florida Complete Care-Duals VIP, Humana Dual Integrated (PPO D-SNP) and UHC Dual Complete FL-Y5.
- C-SNP (Chronic Condition): for people with a qualifying chronic condition. The new Gold Heart & Diabetes Plus is a C-SNP.
- I-SNP (Institutional): for people who live in a long-term care setting or need that level of care. The two new Align Senior Care plans — Premier Care and Senior Care — are I-SNPs.
If you do not meet the SNP's eligibility criteria, you cannot enroll in that plan even if it is open for 2027.
How a brand-new plan is different from a renewing one
With a renewing plan, you can:
- Read the Annual Notice of Change you received by Sept. 30 and see exactly what is changing for 2027.
- Look up the plan's current Star Rating on medicare.gov.
- Call the member services line and ask about prior authorizations you have had approved.
With a new plan, none of that history exists. You can still see its 2027 premium, deductible, drug formulary, provider directory and Summary of Benefits on the Medicare Plan Finder when it refreshes for 2027 — but there is no multi-year track record yet. Treat the plan documents as the source of truth and read them before you enroll.
