Jacksonville, Fla. — October 2026. As the Annual Enrollment Period opens, Medicare Advantage shoppers in Duval County are looking at a 2027 lineup that is noticeably reshuffled: 15 plans are new, 18 will not return, one carrier is leaving the county and a new one is arriving. If you live in Jacksonville and get your Medicare through an Advantage plan, this is the year to actually open your mail.

The federal government sets Medicare Advantage availability county by county, and Jacksonville sits in Duval County. So when we talk about which plans you can choose from, we are talking about Duval County's roster. Below is what is changing for the plan year that starts January 1, 2027 — and what to do if your plan is one of the ones leaving.

The 2026 lineup, for context

Heading into this enrollment season, Duval County has 90 Medicare Advantage plans from 13 carriers. By plan type, that breaks down to 50 HMOs, 12 HMO-POS plans and 28 PPOs. Forty-four are Special Needs Plans (for people who are dual-eligible for Medicaid, live with certain chronic conditions, or live in an institutional setting), and all 90 include drug coverage.

The carriers active in the county this year are Aetna (CVS Health), American Health Advantage of Florida, Blue Cross Blue Shield (HCSC), Centene (WellCare), Devoted Health, Elevance Health (Anthem), Florida Blue, Florida Blue HMO, Florida Complete Care, Gold Kidney Health Plan, Humana, Longevity Health Plan and UnitedHealthcare. The table below lists every plan.

What's new for 2027

Fifteen plans are entering the Duval County market for 2027. They are, listed alphabetically by carrier:

Several of these are Special Needs Plans, so eligibility depends on whether you qualify (for example, having Medicaid as well as Medicare, or a specific chronic condition). The plan's own documents spell out who can enroll.

What's not returning after 2026

Eighteen plans will not be offered in Duval County in 2027. The full list, alphabetical by carrier:

A note on the names: a few plans leaving the market share a name with a plan that's new for 2027 (for example, "Wellcare Sunshine Health Dual Align" and "Florida Complete Care-Duals VIP"). Those are different plan contracts under the same name — your current one is ending, and a new one with the same name is beginning. Read the paperwork carefully before assuming nothing has changed.

If your plan is leaving, you have options

When a Medicare Advantage plan is not renewed in your area, Medicare opens a Special Enrollment Period for the people in it. In practical terms, that means you can:

Your plan is required to send you a non-renewal notice. Keep it — it is proof of your SEP. You can also call 1-800-MEDICARE or use medicare.gov to confirm the window that applies to you.

If your plan is simply changing — renamed, or different in premium, deductible or plan type — you do not get that same SEP. You use the regular Annual Enrollment Period, October 15 through December 7, 2026. Seventeen plans in the county are being renamed and 31 are changing on premium, deductible, type or name, so the Annual Notice of Change ("ANOC") your plan mailed in September is worth re-reading.

What else is changing for 2027

Two program-wide figures are confirmed for Part D next year, and they apply to drug coverage inside any Medicare Advantage plan that includes it (all 90 in Duval County do):

The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, had not yet been announced by CMS as of this writing — those usually come out in November. Be cautious of anyone quoting them as final before then.

The optional Medicare Prescription Payment Plan, which lets you spread your drug costs across the calendar year in monthly bills instead of paying at the pharmacy counter, continues in 2027. You can opt in through your plan.

How to use the next few weeks

A few steps that make the shopping easier:

1. Find your ANOC (mailed by your current plan in September). It tells you what is changing on your specific plan for 2027. 2. List your doctors, pharmacies and regular prescriptions. Networks and drug formularies change every year — a plan that worked in 2026 may not cover the same things in 2027. 3. Use the Medicare Plan Finder at medicare.gov. It is the official tool, it is free, and it shows actual 2027 premiums, deductibles, drug costs and star ratings side by side. 4. If you do nothing and your plan is being offered again next year, you'll stay in it automatically with the changes described in the ANOC. If your plan is on the "leaving" list above, doing nothing means you'll be back in Original Medicare on January 1, 2027 without drug coverage.

If your current Medicare Advantage plan is being offered again in 2027, you'll stay in it, with whatever changes are described in your Annual Notice of Change. If your plan is one of the 18 leaving Duval County, you'll return to Original Medicare on January 1, 2027 unless you pick a new plan — but you'll have a Special Enrollment Period to make a choice.
Is Blue Cross Blue Shield really leaving Jacksonville Medicare Advantage?
