TALLAHASSEE — Floridians who have both Medicare and Medicaid will see 130 Dual Special Needs Plans (D-SNPs) on the 2027 Medicare Advantage menu, out of 560 Medicare Advantage plans offered statewide, according to the CMS Medicare Advantage Landscape Source Files for the coming plan year. D-SNPs are one slice of the Special Needs Plan world; the other two slices in Florida for 2027 are Chronic Condition SNPs (110) and Institutional SNPs (12).

What a D-SNP is and who qualifies

A Dual Special Needs Plan is a type of Medicare Advantage plan built specifically for people who have both Medicare and Medicaid. The plans coordinate the two programs so a member uses one Medicare Advantage plan for Part A, Part B and (in most cases) Part D drug coverage, while Medicaid continues to cover what it covers for that person under Florida rules.

Dual eligibility comes in two broad flavors. "Full" dual eligibility means Medicaid helps with Medicare cost-sharing and also provides full Medicaid benefits. "Partial" dual eligibility — for example, people in the Qualified Medicare Beneficiary (QMB) category — means Medicaid helps with Medicare premiums and cost-sharing but does not provide full Medicaid benefits. Different D-SNPs are designed around these different categories. You will see this reflected in plan names on the 2027 list, which include "Full Dual," "Partial Dual" and "QMB Only" labels from carriers such as Aetna (CVS Health), Devoted Health and Doctors HealthCare Plans, Inc.

Eligibility for Medicaid — and the specific category you fall into — is determined and confirmed by Florida's state Medicaid agency. A D-SNP cannot enroll you based on self-report alone; it verifies your Medicaid status with the state.

The statewide count and the carriers offering them

For 2027, 14 carriers are offering D-SNPs somewhere in Florida. In alphabetical order, with the number of D-SNP plan entries each has statewide: Aetna (CVS Health) (23), AmeriHealth Caritas VIP Care (HMO-D-SNP) (2), Blue Cross Blue Shield (HCSC) (7), Centene (WellCare) (6), Devoted Health (29), Doctors HealthCare Plans, Inc. (4), Elevance Health (Anthem) (9), Florida Complete Care (3), Humana (23), Leon Health, Inc. (2), Prominence Health Plan (1), Solis Health Plans (5), Ultimate Health Plans (2), and UnitedHealthcare (14).

Those counts are statewide totals. Because D-SNPs are sold county by county, a carrier with many plan entries may still cover only a limited set of counties, and a carrier with few entries may be exactly what is offered where you live.

Among the 2027 filings, some plans are new and some have been renamed. New entries include several from Centene (WellCare) under the Wellcare Sunshine Health name, new "Partial Dual" plans from Aetna (CVS Health), new AmeriHealth Caritas VIP Care offerings, new Devoted Health "DUAL FULL" plans, and new "DrPartialDual" plans from Doctors HealthCare Plans, Inc. Renamed plans include Aetna's "QMB Only Select" and "Partial Dual Select" lines (previously grouped under Aetna Medicare Dual Select), several Devoted Health "DUAL QMB" plans (previously "DUAL PLUS"), and Wellcare Sunshine Health Dual Reserve (previously Wellcare Dual Reserve). How often a dual-eligible person can change plans

Most people with Medicare can change Medicare Advantage or Part D plans during the Annual Enrollment Period, October 15 – December 7, 2026, for a January 1, 2027 effective date. Dual-eligible beneficiaries have more flexibility.

People with both Medicare and Medicaid generally qualify for a Special Enrollment Period that lets them make a change once per calendar quarter during the first three quarters of the year (January–March, April–June, July–September). Changes made under that SEP take effect the first day of the following month. The Medicare Advantage Open Enrollment Period, January 1 – March 31, 2027, also allows a one-time switch from one Medicare Advantage plan to another, or back to Original Medicare. Rules can change, so the authoritative source for enrollment periods is medicare.gov.

If a person's Medicaid status changes — for example, they lose Medicaid or move from one category to another — a separate Special Enrollment Period may apply. The state Medicaid agency is the office that confirms current status.

The simplest way to see which of the 130 Florida D-SNPs are actually offered at your address is to use the Medicare Plan Finder at medicare.gov. Enter your ZIP code and, when prompted, indicate that you have Medicaid. The tool will then narrow results to the D-SNPs available in your county, and you can compare premiums, cost-sharing, drug coverage, provider networks and star ratings side by side.
