Miami shoppers have a menu to work through this fall. For plan year 2027, Miami-Dade County has 81 Medicare Advantage plans on offer from 16 carriers, and every single one of them includes prescription drug coverage. The table below lists each plan, its carrier and its plan type, so you can see the whole field on one page instead of piecing it together from mailers.
Key takeaways
- Miami-Dade County has 81 Medicare Advantage plans for 2027 from 16 carriers, and all of them include Part D drug coverage.
- The lineup breaks down into 85 HMO plans, 9 HMO-POS plans and 21 PPO plans, and 69 of the 115 are Special Needs Plans.
- No stand-alone Part D plans are listed in the county's 2027 Advantage roster, so drug coverage for MA enrollees comes bundled with the plan.
- The Annual Enrollment Period runs October 15 through December 7, and the Medicare Advantage Open Enrollment Period runs January 1 through March 31.
Because Medicare Advantage availability is set at the county level, the same lineup applies whether you live in Little Havana, Kendall or North Miami. Miami is in Miami-Dade County, where CMS's 2027 Landscape file shows a mix of HMO, HMO-POS and PPO plans, including Special Needs Plans (SNPs) for people with Medicaid, certain chronic conditions or who live in institutional settings.
What's on the menu in Miami-Dade for 2027
Of the 81 plans, 65 are continuing from 2026, 16 are new, 30 did not return and 20 were renamed; 41 continuing plans change something in their terms, with the details in each plan's Annual Notice of Change. HMOs generally ask you to use an in-network group of doctors and hospitals and get referrals for specialists; HMO-POS plans loosen that up in specific situations; PPOs let you go out of network, usually at a higher cost. Every plan in this county's 2027 roster bundles Medicare Part D drug coverage, which is why you'll see the note "with drug coverage" on every row of the table.
Sixty-nine of the plans are Special Needs Plans. Those are only for people who qualify — typically because they have both Medicare and Medicaid (D-SNP), a qualifying chronic condition like diabetes, heart failure or kidney disease (C-SNP), or live in a nursing facility (I-SNP).
The carriers offering plans in Miami-Dade
Sixteen carriers are on the 2027 Miami-Dade landscape. Listed alphabetically, they are: Aetna (CVS Health), Align Senior Care, AmeriHealth Caritas VIP Care, Centene (WellCare), Devoted Health, Doctors HealthCare Plans, Elevance Health (Anthem), Florida Blue, Florida Blue HMO, Florida Complete Care, Gold Kidney Health Plan, Humana, Leon Health, Longevity Health Plan, Solis Health Plans and UnitedHealthcare. Blue Cross Blue Shield (HCSC) is not offering plans in the county for 2027.
Humana lists the most plans in the county at 32, followed by UnitedHealthcare at 17 and Elevance Health (Anthem) at 15. Smaller lineups include Longevity Health Plan and AmeriHealth Caritas VIP Care at one each, and Align Senior Care, Blue Cross Blue Shield (HCSC) and Florida Blue at two. The full carrier-by-carrier breakdown, with every plan name and type, is in the table below.
Plan types, explained in plain English
- HMO: You pick a primary care doctor, use the plan's network and usually need a referral for specialists.
- HMO-POS: An HMO with a "point of service" option — the plan may cover certain services from out-of-network providers under set rules.
- PPO: More flexibility to see out-of-network providers, though you generally pay more when you do.
- SNP (C-SNP, D-SNP, I-SNP): A Special Needs Plan designed around a specific group. You must meet the eligibility rules to join.
Names like "DEVOTED DUAL," "UHC Dual Complete," "Wellcare Dual" or anything marked D-SNP signal a plan for people with both Medicare and Medicaid. C-SNP names often flag the condition — for instance, Gold Kidney Health Plan's C-SNP plans focus on dialysis and kidney conditions, and there are C-SNPs from other carriers built around diabetes, heart disease, lung disease or chronic care.
Drug coverage and the 2027 Part D rules
Every Miami-Dade Advantage plan for 2027 includes Part D. A few program-wide numbers worth knowing when you compare drug coverage:
- The Part D out-of-pocket cap is $2,100 in 2026 and $2,400 in 2027. Once your covered drug spending reaches that amount, you pay nothing for covered drugs the rest of the year.
- The Part D deductible cannot be more than $615 in 2026 and $700 in 2027 (individual plans may set a lower one).
- The old "donut hole" coverage gap is gone as of 2025. Part D now has three phases: deductible, initial coverage and catastrophic.
- The Medicare Prescription Payment Plan is an optional way to spread your drug costs into level monthly payments through the year.
For higher-income enrollees, IRMAA surcharges on Part B and Part D premiums begin above $109,000 in income for a single filer in 2026.
How to use the table
The table lists every 2027 Miami-Dade plan by carrier, with its plan type and drug-coverage status. Treat it as your starting point, not the final word — the table cannot tell you which doctors are in a plan's network, what a plan charges for a specific medication or what its extras look like. For those details:
- Use the Medicare Plan Finder at medicare.gov to enter your ZIP code, drugs and pharmacies and compare specifics.
- Call 1-800-MEDICARE (TTY 1-877-486-2048), open 24/7.
- Reach Florida SHINE, Florida's State Health Insurance Assistance Program (SHIP), for free one-on-one counseling.
Deadlines to keep in mind
The Annual Enrollment Period runs October 15 through December 7. Changes made during that window take effect January 1. If you're already in a Medicare Advantage plan, you also get the Medicare Advantage Open Enrollment Period from January 1 through March 31 to switch to another Advantage plan or return to Original Medicare.
