What a C-SNP is, and who qualifies
A Chronic Condition Special Needs Plan is a type of Medicare Advantage plan built around a specific long-term health condition. To enroll, you must have Medicare Parts A and B, live in the plan's service area, and have the qualifying chronic condition the plan is designed for. CMS requires that the condition be verified by your provider — typically through a form the plan sends to your doctor after you apply. If the condition cannot be verified, you won't be able to stay enrolled in the C-SNP.
