What a C-SNP actually is

How enrollment works — and why the diagnosis matters

A C-SNP is not open to everyone. To join, you need:

The plan has a short window to get that confirmation after you apply. If your provider does not verify the condition, the plan cannot keep you enrolled.

The Annual Enrollment Period runs October 15 through December 7, 2026. Any change you make in that window takes effect January 1, 2027.

If you do nothing, nothing dramatic happens: your current plan renews for 2027 if it is still being offered, with the changes described in your Annual Notice of Change (ANOC). That document arrives by late September — read it.
