CLEVELAND — As the Medicare Annual Enrollment Period opens on October 15, 2026 and runs through December 7, 2026, people in Cleveland who live with a serious long-term illness have a specific set of Medicare Advantage plans built for them: Chronic Condition Special Needs Plans, or C-SNPs. For plan year 2027, Cuyahoga County — the county Cleveland sits in — has 13 of these plans on the CMS landscape, offered by six carriers. Joining one is not just a matter of signing up; a provider has to confirm the qualifying diagnosis first.

What a C-SNP actually is

A Chronic Condition Special Needs Plan is a type of Medicare Advantage plan. Like other Medicare Advantage plans, it bundles your Part A (hospital) and Part B (medical) benefits, and all C-SNPs in Cuyahoga County for 2027 also include Part D drug coverage.

The Special Enrollment Period you might not know about

Most people change Medicare Advantage plans during the Annual Enrollment Period (October 15 – December 7, 2026), with new coverage starting January 1, 2027. But people with a qualifying chronic condition get an extra option.

Qualifying for a C-SNP opens a Special Enrollment Period. That means you can enroll in a C-SNP outside of the regular October–December window — the SEP is available once you have the qualifying diagnosis. The specific timing rules are on medicare.gov, and the plan can walk you through them.

If you do nothing during AEP, your current plan simply renews for 2027 — assuming it's being offered next year — with whatever changes are described in its Annual Notice of Change (ANOC) that arrives in the mail each fall. In Cuyahoga County for 2027, 31 plans are not returning, 18 are new, and 6 have been renamed, so the ANOC is worth reading.

What's confirmed for 2027, and what isn't

A few program-wide figures are already set. For 2027, the Part D out-of-pocket drug cap is $2,400 (up from $2,100 in 2026), and the Part D maximum deductible is $700 (up from $615 in 2026). Every C-SNP in Cuyahoga County includes Part D coverage, so those caps apply.
