Humana's 2027 Medicare Advantage plans in Honolulu County: three renames and how to double-check yours
Honolulu — Fall 2026. If you have a Humana Medicare Advantage plan on Oʻahu, the paperwork landing in your mailbox this fall may look a little different. For plan year 2027, Humana is keeping the same number of Medicare Advantage plans in Honolulu County — six — but three of them are being offered under new names. No Humana plans are leaving the county, and no brand-new Humana plans are being added here.

Area counts are for the county as a whole and are not a recommendation of any plan. Program figures are set nationally.
That matters because the Annual Enrollment Period runs October 15 through December 7, 2026, and any change you make takes effect January 1, 2027. If you do nothing, your current plan simply renews — under its new name, if it has one — with whatever changes are spelled out in its Annual Notice of Change (ANOC).
MyMedicarePlans.org is an independent consumer site. We are not Humana, and we are not Medicare. Humana is one of seven carriers offering Medicare Advantage in Honolulu County for 2027; across all carriers, 29 plans are offered countywide.
What Humana Medicare Advantage plans are offered in Honolulu County for 2027?
Humana is offering six Medicare Advantage plans in Honolulu County for 2027: one HMO and five PPOs. Five of the six include Part D prescription drug coverage; one PPO does not. None of the six are Special Needs Plans (no D-SNP, C-SNP or I-SNP from Humana here).
The full roster — spelled exactly as CMS lists them, in alphabetical order — appears in the table on this page. The 2027 Humana lineup in the county is:
- Humana Essentials Plus Giveback (PPO) — with drug coverage
- Humana Total Complete (HMO) — with drug coverage
- Humana USAA Honor Giveback (PPO) — without drug coverage
- Humana Value Choice (PPO) — with drug coverage
- HumanaChoice H5216-313 (PPO) — with drug coverage
- HumanaChoice H5216-464 (PPO) — with drug coverage
Because Honolulu is in Honolulu County, these are the Humana plans available anywhere on Oʻahu for 2027. We can't quote premiums, copays, networks or star ratings here — those are plan-specific and belong on medicare.gov or in each plan's own documents.
What's new, what's not returning, and what's renamed for 2027?
In Honolulu County, no Humana plans are leaving for 2027 and no new Humana plans are being added. Three continuing plans, however, are being offered under new names.
The three renamed plans for 2027 are:
- Humana Essentials Plus Giveback (PPO) — previously Humana USAA Honor Giveback with Rx (PPO)
- Humana Total Complete (HMO) — previously Humana Gold Plus H0028-082 (HMO)
- Humana Value Choice (PPO) — previously HumanaChoice H5216-465 (PPO)
A rename isn't just cosmetic — the plan's benefits, cost sharing, drug list or network can also shift year to year. The Annual Notice of Change (ANOC) Humana mailed in September spells out what is different for your specific plan in 2027.
How do I check my doctors and prescriptions before December 7?
The quickest way is the Medicare Plan Finder at medicare.gov. You can enter your Honolulu ZIP code, add your prescriptions and your pharmacy, and see how each plan handles them. You can also call 1-800-MEDICARE (TTY 1-877-486-2048), 24 hours a day, seven days a week.
A few things worth doing before December 7, 2026:
1. Read your ANOC. It lists every change to your current plan for 2027 — including any name change. 2. Re-check your prescriptions. Drug formularies and tiers can change each year, even if the plan name hasn't. 3. Re-check your doctors and hospitals. Provider networks are set by each plan and can change. For HMOs, in-network care generally matters more than for PPOs. Confirm directly with your doctor's office that they will accept the specific plan in 2027. 4. Check your pharmacy. Preferred pharmacies can differ by plan.
What should I know about Part D costs for 2027?
Two Part D numbers are already confirmed for 2027 by CMS:
- The Part D out-of-pocket cap is $2,400 in 2027, up from $2,100 in 2026. Once you've paid that much out of pocket for covered drugs, you pay $0 for the rest of the calendar year.
- The Part D maximum deductible is $700 in 2027, up from $615 in 2026. Individual plans can set a lower deductible.
The coverage gap (the old "donut hole") was eliminated in 2025, so Part D now has three phases. The Medicare Prescription Payment Plan also remains available in 2027 — it lets you spread your out-of-pocket drug costs across monthly payments instead of paying at the pharmacy counter.
CMS has not yet announced the 2027 Part B standard premium and deductible or the 2027 IRMAA income brackets. Those usually come out in the fall, typically November. Until then, don't rely on projections.
What happens if I do nothing by December 7, 2026?
If you take no action, your current plan rolls over into 2027 as long as it's being offered next year — including under a new name, in the case of the three renamed Humana plans above. The changes listed in your ANOC become your coverage on January 1, 2027.
If you later decide the renewed plan isn't the right fit, Medicare Advantage members have a second window: the Medicare Advantage Open Enrollment Period from January 1 to March 31, 2027. During that window you can switch to another Medicare Advantage plan or go back to Original Medicare (and add a stand-alone Part D plan). It's a narrower window than AEP, with its own rules, but it exists.
