MyMedicarePlans.org is an independent consumer news and education site. We are not Wellcare, and Wellcare is one of 16 carriers offering Medicare Advantage plans in Cuyahoga County for 2027 (99 plans in all).

What Medicare Advantage plans does Wellcare offer in Cleveland for 2027?

In Cuyahoga County — the county Cleveland sits in — Wellcare offers four Medicare Advantage plans for 2027. All four are HMO plans, all four are Dual Eligible Special Needs Plans (D-SNPs), and all four include Part D prescription drug coverage. Wellcare is not offering any C-SNP or I-SNP plans in the county for 2027.

The table below lists every Wellcare plan available in the county for next year. In alphabetical order, they are:

Because every Wellcare plan in Cuyahoga County for 2027 is a D-SNP, eligibility is limited to people who qualify for both Medicare and Medicaid. If you are not dual-eligible, Wellcare's 2027 Cleveland menu will not fit you, and you would want to look at the other 15 carriers in the county.

What's new and what's not returning in Wellcare's Cleveland lineup?

Three Wellcare plans are new in Cuyahoga County for 2027: Wellcare Buckeye Health Dual Access (HMO D-SNP), Wellcare Buckeye Health Dual Reserve (HMO D-SNP), and Wellcare Buckeye MyCare Ohio Dual Align (HMO D-SNP). One continuing plan, Wellcare Buckeye MyCare Ohio Dual Align Unity (HMO D-SNP), is a rename of the 2026 Wellcare Buckeye MyCare Ohio Dual Align (HMO D-SNP).

Four Wellcare plans from 2026 are not returning in the county for 2027. If you were enrolled in a plan that is not continuing, Wellcare will send a non-renewal notice explaining your options and the special enrollment rights available to you. You do not have to stay with the same carrier — you can switch to another Medicare Advantage plan or return to Original Medicare and add a stand-alone Part D plan.

What happens if I do nothing before December 7, 2026?

If your current plan is being offered again for 2027, it renews automatically with the changes described in its Annual Notice of Change. Nothing happens to someone who takes no action — the coverage simply rolls over on January 1, 2027. If your plan is one of the four Wellcare plans not returning, you need to act, because that coverage ends December 31, 2026.

The Annual Enrollment Period runs October 15 through December 7, 2026. After that, Medicare Advantage members have one more window — the Medicare Advantage Open Enrollment Period, January 1 through March 31, 2027 — to switch Advantage plans or drop back to Original Medicare.

How do I check a Wellcare plan against my doctors and prescriptions?

Before you enroll in or stay in any plan, confirm three things: your doctors are in the plan's network, your pharmacy is in the plan's network, and every medication you take is on the plan's formulary at a tier you can afford. The fastest way is medicare.gov's Plan Finder, which lets you enter your drugs and ZIP code and see each plan's drug list, pharmacy network and estimated yearly cost.

For a definitive answer, read the plan's Evidence of Coverage and provider and pharmacy directories, or call Wellcare directly using the number on its plan materials. You can also call 1-800-MEDICARE (TTY 1-877-486-2048), 24 hours a day, 7 days a week.

What 2027 Medicare figures are already confirmed?

A few program-level numbers for 2027 are set. The Part D out-of-pocket cap rises to $2,400 in 2027 (up from $2,100 in 2026). The Part D maximum deductible a plan may charge is $700 in 2027 (up from $615 in 2026). The Part D national base beneficiary premium is $41.33 for 2027 — that is a formula input Medicare uses, not what any specific plan charges.

The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet; CMS typically publishes those figures later in the fall. The coverage gap, or "donut hole," has been eliminated since 2025, and the Medicare Prescription Payment Plan remains available to spread drug costs across the year in monthly installments.

Frequently asked questions

Can anyone join a Wellcare plan in Cleveland for 2027?
No. All four Wellcare Medicare Advantage plans in Cuyahoga County for 2027 are Dual Eligible Special Needs Plans (D-SNPs), which means enrollment is limited to people who have both Medicare and Ohio Medicaid. If you are not dual-eligible, you would need to look at a different plan or carrier in the county.
What should I do if my 2026 Wellcare plan is not returning in 2027?
Watch for the non-renewal notice Wellcare is required to send. You then have a Special Enrollment Period to choose another Medicare Advantage plan, enroll in a stand-alone Part D plan with Original Medicare, or make other coverage changes. Use medicare.gov or call 1-800-MEDICARE to compare your options before year-end.
How do I know if a plan covers my doctors and medications?
Use medicare.gov's Plan Finder — enter your ZIP code, your prescriptions and your preferred pharmacy, and the tool will show each plan's drug list and network status. For a final check, read the plan's Evidence of Coverage and current provider and pharmacy directories, or call the carrier directly.
When do 2027 coverage changes take effect?
Any plan change you make during the Annual Enrollment Period (October 15 – December 7, 2026) starts January 1, 2027. If you do nothing and your plan continues, it renews automatically for 2027 with the changes outlined in its Annual Notice of Change. For personalized help comparing plans in Cuyahoga County, visit medicare.gov or call 1-800-MEDICARE (TTY 1-877-486-2048), 24 hours a day, 7 days a week.
