Does Anthem drop any Cleveland Medicare plans for 2027?

CLEVELAND — Anthem's 2027 Medicare Advantage menu in Cuyahoga County is set, and it looks a little different than the one Cleveland shoppers saw a year ago. The carrier (which CMS groups under Elevance Health) is offering 11 Medicare Advantage plans in the county for the plan year that starts January 1, 2027 — two of them brand new, one renamed, and none of its current plans leaving the market. Any changes a reader makes during the Annual Enrollment Period, October 15 through December 7, 2026, take effect on New Year's Day.

MyMedicarePlans.org is an independent consumer site; we are not Anthem and not Medicare. Anthem is one of 16 carriers selling Medicare Advantage in Cuyahoga County next year, and together those carriers offer 99 plans. This piece walks through just Anthem's slice so you can see what is on the shelf before you compare.

What Medicare Advantage plans does Anthem offer in Cleveland for 2027?

Anthem is offering 11 Medicare Advantage plans in Cuyahoga County, the county Cleveland sits in, for plan year 2027. By type, that breaks down to 4 HMO plans, 2 HMO-POS plans and 5 PPO plans. The table below lists every Anthem plan by name and type so you can see the full roster.

Nine of the 11 plans include Part D prescription drug coverage built in. The other two — the Anthem Veteran (PPO) and Anthem Veteran (Regional PPO) — are offered without drug coverage, a design often used by people who already get drug coverage through the VA.

Which Anthem plans are Special Needs Plans in Cuyahoga County?

Four of Anthem's 11 plans in Cuyahoga County are Dual-Eligible Special Needs Plans (D-SNPs), built for people who have both Medicare and Medicaid. Anthem is not offering a Chronic Condition SNP (C-SNP) or an Institutional SNP (I-SNP) in the county for 2027.

The four D-SNPs are Anthem Dual Advantage (HMO D-SNP), Anthem Dual Advantage Plus (HMO D-SNP), Anthem I MyCare Ohio Full Dual Advantage (HMO D-SNP) and Anthem I MyCare Ohio Full Dual Support (HMO D-SNP). All four include Part D drug coverage. To join a D-SNP, you have to qualify for Medicaid as well as Medicare.

What is new or renamed in Anthem's 2027 Cleveland lineup?

Anthem is adding two plans and renaming one in Cuyahoga County for 2027, and none of its 2026 plans are being discontinued. The new plans are Anthem Dual Advantage Plus (HMO D-SNP) and Anthem I MyCare Ohio Full Dual Advantage (HMO D-SNP). The renamed plan is Anthem I MyCare Ohio Full Dual Support (HMO D-SNP), which was previously called Anthem I MyCare Ohio Full Dual Advantage (HMO D-SNP).

If you are already enrolled in an Anthem plan, your Annual Notice of Change (ANOC) — mailed in late September — spells out exactly what is changing in your plan for 2027, including any cost or benefit updates. If you do nothing, your current plan rolls over on January 1, 2027 with the changes the ANOC describes.

How do I check an Anthem plan against my doctors and prescriptions before December 7?

The quickest way is the Medicare Plan Finder at medicare.gov. You enter your ZIP code, add the prescriptions you take (name, dose and how often), and add your preferred pharmacy. The tool then shows each plan's estimated yearly drug cost at that pharmacy and whether each drug is on the plan's formulary.

For doctors and hospitals, call Anthem directly or look at the plan's online provider directory — networks differ from plan to plan even within the same carrier, so a doctor who is in-network on one Anthem plan may not be in-network on another. For a second opinion or help over the phone, call 1-800-MEDICARE (TTY 1-877-486-2048), which is open 24/7. The deadline to make a change for January 1, 2027 coverage is December 7, 2026.

What should I know about 2027 Medicare costs before I compare?

CMS has confirmed a few program-level numbers for 2027 that affect any plan with drug coverage. The Part D out-of-pocket cap rises to $2,400 in 2027 (up from $2,100 in 2026). The Part D maximum deductible a plan can charge rises to $700 in 2027 (up from $615 in 2026). The Part D national base beneficiary premium for 2027 is $41.33 — that is a formula input, not a dollar figure any single plan charges.

The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet. CMS typically publishes them later in the fall, usually in November. We will update our coverage as soon as those numbers are out.

Frequently asked questions

Do I have to switch plans if I like my current Anthem plan?
No. If your plan is being offered again in 2027, it renews automatically on January 1, 2027. Read your Annual Notice of Change to see what, if anything, is changing. None of Anthem's 2026 plans in Cuyahoga County are being discontinued for 2027.
What is the difference between Anthem's HMO, HMO-POS and PPO plans?
An HMO generally asks you to use in-network providers and often requires a referral to see a specialist. An HMO-POS (point-of-service) adds some out-of-network flexibility, usually at higher cost. A PPO lets you see out-of-network providers, typically at higher cost-sharing. The specific rules, networks and costs vary by plan, so check each plan's Evidence of Coverage.
Can I join one of Anthem's D-SNPs if I only have Medicare?
No. Dual-Eligible Special Needs Plans are limited to people who qualify for both Medicare and Medicaid. If you are not sure whether you qualify for Medicaid in Ohio, call 1-800-MEDICARE or check with the Ohio Department of Medicaid.
Where can I see every carrier's plans, not just Anthem's?
Use the Medicare Plan Finder at medicare.gov and enter your Cuyahoga County ZIP code. It will show all 99 Medicare Advantage plans from the 16 carriers offering coverage in the county for 2027, so you can compare Anthem's plans side by side with the others.
