Humana Medicare Advantage in Illinois for 2027: 32 Plans, County by County
SPRINGFIELD, Ill. — For the 2027 plan year, Humana is listing 32 Medicare Advantage plans in Illinois, according to the Centers for Medicare & Medicaid Services (CMS) Medicare Advantage Landscape Source Files. The state has 11 other Medicare Advantage carriers for the year, and together all carriers offer 139 plans. Because every Medicare Advantage plan is approved county by county, a plan on Humana's statewide list may or may not be sold where you live. This explainer, from an independent Medicare education site not affiliated with the government or any insurance carrier, walks through what the CMS files show.

What Humana offers statewide for 2027
Across Illinois, Humana's 2027 Medicare Advantage lineup from the CMS Landscape files comes to 32 plans. By plan type, that's 10 HMOs, 20 PPOs, and 2 PFFS (Private Fee-for-Service) plans. All 32 are listed as plans that include Medicare prescription drug coverage.
Three of those plans are Special Needs Plans (SNPs), which serve people who meet specific eligibility rules:
- 1 Dual Eligible Special Needs Plan (D-SNP), for people with both Medicare and Medicaid.
- 2 Chronic Condition Special Needs Plans (C-SNPs), which in Humana's Illinois lineup are oriented to people living with diabetes and heart conditions.
- 0 Institutional Special Needs Plans (I-SNPs).
Humana is one of 12 Medicare Advantage carriers in Illinois for 2027, out of a statewide total of 139 Medicare Advantage plans across all carriers. The full plan-by-plan list on this page is rendered as a table from the same CMS data.
Humana plans with the widest county reach
"Widest county reach" simply means the number of Illinois counties where a plan is approved to be sold in 2027, per CMS. It is not a quality rating.
At the top of Humana's Illinois footprint for 2027 are two plans listed in 102 counties each: Humana Dual Fully Integrated (HMO D-SNP) and Humana USAA Honor Giveback (Regional PPO), along with HumanaChoice R5361-002 (Regional PPO), also at 102 counties. Three other PPOs show broad reach: Humana USAA Honor Giveback (PPO) at 93 counties, HumanaChoice Giveback H5216-403 (PPO) and HumanaChoice H5216-399 (PPO) at 87 counties each, and HumanaChoice Giveback H7617-013 (PPO) at 85 counties. On the PFFS side, Humana Gold Choice H8145-006 (PFFS) is listed in 48 counties and Humana Gold Choice H8145-126 (PFFS) in 41.
Other Humana plans have a narrower county footprint, including the Humana Community HMO Diabetes and Heart (HMO C-SNP) at 3 counties and HumanaChoice H7617-007 (PPO) at 1 county. The accompanying table on this page shows every plan and its 2027 county count.
Remember: even a plan listed in 102 counties will not be sold in all 102 of Illinois's 102 counties if your county is outside its service area, and a plan sold statewide may still not be offered where you live. Always confirm by county.
What's new and what's not returning
Comparing the CMS Landscape files for 2026 and 2027, Humana has:
- 4 plans new for 2027 in Illinois, including Humana Gold Plus - Diabetes and Heart (HMO C-SNP), a new Humana Essentials Plus Giveback (HMO), a new Humana Total Complete (HMO), and HumanaChoice H7617-007 (PPO).
- 4 plans that are not returning for 2027.
Several familiar plans have been renamed. For example, plans previously sold as Humana Gold Plus Giveback are now listed as Humana Essentials Plus Giveback (HMO); some Humana Gold Plus HMOs are now Humana Total Complete (HMO); certain Humana Full Access (PPO) and HumanaChoice (PPO) plans have moved to the Humana Full Access or Humana Value Choice naming. A rename does not by itself tell you anything about costs, benefits, provider networks or drug lists — those details live in each plan's Evidence of Coverage.
If you are currently in a Humana plan, watch your mail in fall 2026 for an Annual Notice of Change (ANOC), which spells out any changes for the coming year.
Checking your county, your doctors, and your prescriptions
Three questions matter more than any statewide count:
1. Is the plan offered in my county in 2027? Use the Medicare Plan Finder at medicare.gov and enter your ZIP code, or call 1-800-MEDICARE (TTY 1-877-486-2048), available 24/7. 2. Are my doctors, hospitals, and other providers in the plan's network? Check the plan's own provider directory, and confirm with each provider's office. 3. Are my prescriptions on the plan's formulary, and at what tier and pharmacy? Check the plan's formulary and pharmacy directory.
These details come from the Medicare Plan Finder, 1-800-MEDICARE, or the plan's own Evidence of Coverage and directories — not from the CMS Landscape counts used in this article.
What to do before December 7
The Medicare Annual Enrollment Period runs October 15 – December 7, 2026. Any Medicare Advantage or Part D choice you make in that window takes effect January 1, 2027. A simple checklist:
- Read your current plan's Annual Notice of Change if you have one.
- List your doctors, hospitals, and prescriptions, including dosages.
- On medicare.gov, compare plans offered in your ZIP code for 2027.
- Confirm network and formulary details directly with the plan.
- Keep a written confirmation number for any enrollment you complete.
If you don't want to make a change, you don't have to; most Medicare Advantage plans renew automatically if they are returning.
