INDIANAPOLIS — As the Annual Enrollment Period opens for 2027 coverage, Marion County shoppers will see a Medicare Advantage menu that leans heavily toward one plan type. Of the 79 Medicare Advantage plans offered in the county for 2027, 42 are PPOs, 22 are HMOs and 15 are HMO-POS, according to the CMS Medicare Advantage Landscape Source Files. No PFFS plans are on the 2027 lineup for Marion County — a shift from the current plan year.

That split matters because the letters after a plan name are a shortcut for how it handles doctors, referrals and care you get outside the network. Here is what each one means, and which carriers are offering which type next year.

What each plan type actually means

Plan type is a program-level rule set, not a brand. These descriptions apply to Medicare Advantage anywhere, not to any specific plan sold here.

HMO (Health Maintenance Organization). You pick a primary care doctor and generally use providers in the plan's network. Many HMOs require a referral to see a specialist. Care outside the network is typically not covered except for emergencies, urgent care and out-of-area dialysis.

HMO-POS (HMO with a Point-of-Service option). It is an HMO with a limited out-of-network door. The plan decides which services you can get out of network and at what cost share. Referral rules still vary by plan.

PPO (Preferred Provider Organization). You can see any Medicare-approved provider who will take the plan. In-network care usually costs less than out-of-network care, and most PPOs do not require referrals to see a specialist.

PFFS (Private Fee-for-Service). These plans set the terms providers must accept to see you. Any provider who agrees to those terms at the time of your visit can treat you. No PFFS plans are being offered in Marion County for 2027.

What is changing in Marion County for 2027

Compared with the current plan year, the 2027 lineup has 15 new plans, 14 plans not returning and 7 renamed. The total goes from 78 plans this year to 79 next year, and the carrier count stays at 11.

If you already have a Medicare Advantage plan, read the Annual Notice of Change (ANOC) your insurer mails in late September. It spells out what is different about your plan for 2027 — the network, the drug list, the cost shares and any benefit changes. If your plan is one that is not returning, you will get a non-renewal notice and a Special Enrollment Period to pick something else.
