Here is what the numbers mean, what to look for in the mail, and the dates to put on the refrigerator.
What the statewide numbers mean
CMS publishes a Landscape Source File each year that lists every Medicare Advantage plan offered in every county. Comparing the CY2026 and CY2027 files side by side is how we get these counts. A plan is counted once statewide no matter how many Kentucky counties it serves.
A plan is "leaving" when it was offered somewhere in Kentucky for 2026 and is offered nowhere in Kentucky for 2027. By that definition, 21 Kentucky plans are leaving. A plan is "changing" when it appears in both years but differs on premium, drug deductible, plan type, or plan name in at least one county. By that definition, 23 Kentucky plans are changing. Another 30 plans are new for 2027, and 70 continue unchanged.
On the carrier side, the CMS files show Provider Partners Health Plans with no Kentucky plans listed for 2027, and American Health Advantage of Indiana entering Kentucky for 2027.
If your plan is leaving
If your current Medicare Advantage plan is among the 21 leaving Kentucky for 2027, you will get a non-renewal notice from your plan. You have two protections built into Medicare's calendar.
First, the Annual Enrollment Period (AEP) runs October 15 through December 7, 2026. During AEP you can choose any plan available in your county for 2027, and your choice takes effect January 1, 2027.
Second, if your plan is not being renewed, you also get a Special Enrollment Period (SEP) that runs through the end of February 2027. That gives you extra time past December 7 to pick a replacement if you need it.
If you do nothing and your plan is non-renewed, you will not automatically land on a comparable plan. Read the notice from your carrier carefully and act within the SEP window.
If your plan is changing
If your plan is among the 23 that continue but change in at least one county, the specifics live in your Annual Notice of Change, or ANOC. Carriers send the ANOC in the fall before AEP. It spells out what is different about your plan for 2027 compared to 2026 in the county where you live.
If you do nothing, you stay in your plan with the changes described in your ANOC. That can be the right call, or it can be a reason to shop — but you cannot tell without reading the ANOC. The point is simple: open the envelope, and compare it to how you actually used your plan this year.
The counties with the most change
Because plans vary county by county, the same statewide reshuffle lands differently in different places. Using the CMS files, here are the counties the Landscape data flags with the highest leaving and changing counts:
- Barren County: 14 leaving, 13 changing
- Jefferson County: 13 leaving, 14 changing
- Knox County: 14 leaving, 13 changing
- Lewis County: 14 leaving, 13 changing
- Russell County: 14 leaving, 13 changing
- Wayne County: 14 leaving, 13 changing
- Boone County: 13 leaving, 13 changing
- Bullitt County: 12 leaving, 14 changing
If you live in one of these counties, odds are higher that something in your mailbox this fall will matter. But remember: all 120 Kentucky counties see at least one plan leave or change for 2027.
What to do before December 7
A short checklist for Kentucky beneficiaries:
1. Watch for two pieces of mail from your current carrier this fall — the Annual Notice of Change (if your plan is continuing) or a non-renewal notice (if your plan is leaving). 2. Read it against how you actually used the plan in 2026: your doctors, your hospitals, your pharmacy, your prescriptions. 3. Compare plans available in your county for 2027 on Medicare.gov's Plan Finder, which draws on the same CMS Landscape and Plan Finder data. 4. Make any change during the Annual Enrollment Period, October 15 through December 7, 2026. Coverage you pick starts January 1, 2027. 5. If your plan is not renewed, you still have the Special Enrollment Period through the end of February 2027.
