Key takeaways

  • Clark County has 72 Medicare Advantage plans for 2027 from 10 carriers, split 37 HMO, 18 HMO-POS and 17 PPO.
  • HMOs usually require in-network care and sometimes a referral; PPOs let you go out of network, typically at a higher share of the cost.
  • No PFFS (Private Fee-for-Service) plans are offered in Clark County for 2027.
  • The Annual Enrollment Period runs October 15 – December 7, 2026; a change you make takes effect January 1, 2027.

Las Vegas, fall 2026 — When the Annual Enrollment Period opens on October 15, 2026, Las Vegas shoppers choosing a Medicare Advantage plan for 2027 will see a lineup that leans heavily toward HMO-style coverage, with a smaller slice of PPOs. Las Vegas sits in Clark County, where 72 Medicare Advantage plans from 10 carriers are being offered for 2027, according to the CMS Medicare Advantage Landscape Source File. The split by plan type matters, because it decides how you use doctors, whether you need referrals, and what happens if you get care out of network.

How the Clark County lineup splits for 2027

For plan year 2027, Clark County's 72 Medicare Advantage plans break down this way:

Grouped together, HMO and HMO-POS plans account for 55 of the 72 — about three out of four. PPOs make up the rest. No PFFS (Private Fee-for-Service) plans are offered locally for 2027, so that type isn't on the table in Las Vegas this year. The table below lists every plan by carrier and type.

What each plan type actually means

The plan type is not a marketing label — it's a set of rules written into the contract about who you can see and how.

HMO (Health Maintenance Organization). You pick a primary care doctor and generally use providers inside the plan's network. Many HMOs require a referral from your primary care doctor before you see a specialist. Out-of-network care is usually not covered except in an emergency or urgent situation. If your doctors are all in-network and you don't travel much, an HMO can be a clean fit.

HMO-POS (HMO with a Point-of-Service option). This is an HMO with a limited door to out-of-network care for certain services. The specifics — which services, at what cost share — vary by plan and are spelled out in the plan's Evidence of Coverage. Think of it as an HMO with a little flexibility built in.

PPO (Preferred Provider Organization). You can see any provider that accepts Medicare and the plan's terms, in or out of network, usually without a referral. Out-of-network care is covered but typically at a higher share of the cost than in-network care. PPOs often make sense for people who split time between states or want to keep options open.

PFFS (Private Fee-for-Service). These plans set their own payment terms, and any provider who accepts those terms on a visit-by-visit basis can treat you. No PFFS plans are being offered in Clark County for 2027.

Which type is "right" depends on your doctors, your prescriptions, where you spend time, and whether you want referrals handled by a primary care doctor or prefer to self-direct. There is no universal better choice.

Which carriers offer each type in Clark County

For plan year 2027, here is who sells what locally:

A few carriers appear in more than one column because they offer different plan types side by side. If you're committed to a PPO, your choices in Clark County come from three carriers; if you're set on an HMO, the field is wider.

The standard monthly Part B premium has climbed from $88.50 in 2006 to $202.90 in 2026. Hover any point for that year’s premium.

What changed from 2026 to 2027

The 2026 Clark County lineup had 74 plans from 12 carriers; for 2027, it's 72 plans from 10 carriers. Within that total, 11 plans are not returning, 9 are new, and 15 have been renamed. If your current plan has a change — a renaming, a benefit shift, or anything else — your Annual Notice of Change (ANOC) spells out exactly what's different. The ANOC is mailed in September; it's worth reading before you decide whether to stay or switch.

Timing and what to do next

The Annual Enrollment Period runs October 15 – December 7, 2026. Any change you make in that window takes effect January 1, 2027. If you do nothing, your current plan simply renews for 2027 — assuming it is being offered next year — with whatever changes the ANOC describes.

A few program figures to keep in mind as you compare: the Part D out-of-pocket cap rises to $2,400 in 2027 (up from $2,100 in 2026), and the maximum Part D deductible a plan can charge is $700 in 2027 (up from $615 in 2026). The 2027 Part B standard premium and deductible, along with the 2027 IRMAA income brackets, have not been published yet; CMS typically announces them later in the fall.

When you compare plans, start with the plan type question — HMO, HMO-POS or PPO — because that sets the ground rules. Then check whether your doctors and preferred pharmacies are in the network, and whether your prescriptions are on the drug list. The Medicare Plan Finder at medicare.gov is the official tool for that, and 1-800-MEDICARE is available 24/7.

Frequently asked questions

Do I need a referral in a Medicare Advantage HMO?
Often, yes. Many HMOs require you to get a referral from your primary care doctor before seeing a specialist. The exact rule is in each plan's Evidence of Coverage, so check that document — some HMOs waive referrals for certain services.
Can I use out-of-network doctors in a PPO?
Yes. PPO plans cover both in-network and out-of-network providers who accept Medicare and the plan's terms. Out-of-network care is usually covered at a higher share of the cost than in-network care, and the specifics are set by each plan.
Are there any PFFS plans in Las Vegas for 2027?
No. According to the CMS Landscape file for 2027, no Private Fee-for-Service plans are being offered in Clark County next year. The 72 Clark County plans are a mix of HMO, HMO-POS and PPO.
What happens if I don't do anything during enrollment?
If your plan is being offered again in 2027, it renews automatically on January 1, 2027, with the changes described in your Annual Notice of Change. If your plan is one of the 11 not returning, you'll get a non-renewal notice and will need to pick a new plan or move to Original Medicare. ## Where to get personal answers Plan type is the starting point, not the finish line. For plan-specific details — networks, drug lists, costs and star ratings — use the Medicare Plan Finder at medicare.gov, call 1-800-MEDICARE (TTY 1-877-486-2048), or read the plan's own Evidence of Coverage and provider directory. Those are the official sources, and they can confirm whether your doctors and prescriptions fit a plan before you enroll.
